Job Description
Catalyst for Change is looking for a dependable, people-friendly Community Health Worker (CHW) to join our team on a full-time basis. If you take pride in keeping things running smoothly, taking initiative to find resources for clients, collaborate with others and coordinate care for patients we want to hear from you. What you'll do: Serve a vital role in providing flexible, on-demand health services and support to rural communities across Montana. This position is designed for adaptable and resourceful professionals who can seamlessly integrate into various settings, providing immediate support where needed, and assisting with both direct community health efforts and operational tasks. What we're looking for: Someone organized, reliable, comfortable with basic technology and electronic health records (EHR) systems, strong attention to detail and communication skills.
