The QP works collaboratively with the CST Team Lead, Peer Support Specialist, other team members, and community providers to help individuals achieve greater stability and independence. The QP assists members with identifying needs, developing practical skills, connecting with community resources, addressing barriers to recovery, and working toward goals identified in their treatment plan. Services may be provided in the member's home, community, or other appropriate settings.

Primary Responsibilities:

Provide person-centered, recovery-focused support to CST members. Assist members with developing independent living and daily functioning skills. Support members with housing, employment, education, transportation, benefits, and other community-resource needs. Help members identify and address barriers affecting stability and community functioning. Coordinate and communicate with CST team members and other service providers. Participate in team meetings, treatment planning, and care coordination. Complete required service notes and other documentation accurately and within required time frames. Assist with crisis prevention, stabilization, and implementation of established safety or crisis plans within the QP's role. Maintain professional boundaries, confidentiality, and compliance with agency policies and applicable behavioral health requirements. Provide services in community settings and travel to meet members as required.

Basic Qualification:

Bachelor's degree in a human services or related field and ability to meet applicable Qualified Professional (QP) requirements for the position.

Pay:

$24.42 - $29.41 per hour
