Job Description
MAJOR FUNCTION
Community Health Workers (CHWs) help patients address the social, medical and community problems that lead to poor health. The goal is to help patients deal with the 'real life' issues that keep them from staying healthy, help with the tasks of getting medical care, and work on health goals by doing things like planning healthy meals or finding time to exercise.
QUALIFICATIONS
Education & Experience:
High school or equivalent. Bachelor's degree preferred. Prior experience as an outreach worker preferred. The Community Health Worker will complete required Community Health Worker training within one year of hire
Certifications/ Licensure:
Valid NJ Driver's License and Proof of Valid Vehicle Insurance required.
Knowledge & Skills:
Long-time resident or high familiarity of the community CHW will be working with which includes good knowledge of the resources of the community. Must be comfortable meeting with members in home and/or community settings. Proficient oral and written communication skills. Ability to carry out written and oral instructions. Ability to exercise judgment in the application of professional services. Comfortable with computer data entry and possess good data entry skills.
