Job Description

Job Brief Full time Memorial Health System of Southwest Oklahoma -LCHC Clinics - Apache, OK 73006
Compensation:
Based on
Experience Education:
High school diploma or equivalent required. Associate or bachelor's degree in public health, social work, human services, or a related field is preferred. Ongoing training in community health practices and health education is advantageous.
Regulatory Requirements:
Valid driver's license and proof of insurance, if the role involves travel. Background check and health screenings may be required depending on employer policies and funding sources. CPR/First Aid certification may be required or preferred.
Responsibilities:
Conduct outreach and education in homes, clinics, community centers, and other local settings. Assist individuals and families in navigating healthcare and social service systems. Provide culturally appropriate health education and information. Help clients access medical, behavioral health, housing, food, and other essential services. Advocate on behalf of individuals and communities to reduce barriers to care. Build trusting relationships with clients to support health behavior change. Coordinate with healthcare providers and social service agencies for follow-up and care management. Document client interactions and maintain accurate records in compliance with privacy standards. Attend and participate in community events, training, and staff meetings.
