Abilene, TX Job Details Part-time | Full-time | Contract From $15 an hour 9 hours ago Qualifications Public speaking High school diploma or GED Driver's License Organizational skills Data collection Community relationship building Full Job Description Community Health Worker (CHW) Scope of Services Purpose Under a contracted agreement with the Abilene-Taylor County Public Health District (ATCPHD), the Community Health Worker (CHW) provides community outreach, health education, resource navigation, and implementation support for grant-funded public health initiatives. The CHW serves as a trusted liaison between ATCPHD and the community while supporting outreach events, collecting required program documentation, and promoting equitable access to health resources. •
NOTE:
This is a contractual 1099 position with a salary range beginning at $15.00 an hour but can vary based on qualifications this can be a part time to full time position based on the candidate's schedule. Community Engagement Conduct outreach activities in locations where community members live, work, learn, play, and worship. Represent ATCPHD professionally at outreach events and community functions. Listen to community members to identify health concerns, barriers, and opportunities for improvement. Communicate community feedback to designated ATCPHD grant personnel. Support relationships with community organizations and partner agencies. Health Education Deliver evidence-based health education using approved curricula and educational materials. Provide culturally appropriate health information to individuals and groups. Promote healthy behaviors and increase awareness of ATCPHD programs and services. Refer community members to appropriate health and community resources when needed. Program Support Support implementation of grant-funded outreach activities and community events. Prepare educational materials, outreach supplies, and event equipment when independently staffing events. Collect attendance records, participant surveys, release forms, success stories, photographs (when appropriate), and other required grant documentation. Assist with setup, breakdown, and logistical support for community outreach events. Documentation and Administrative Responsibilities Maintain accurate documentation of outreach activities and participant encounters. Enter required data into approved systems and spreadsheets. Complete required reports and submit documentation in a timely manner. Utilize Microsoft Office, Canva, AI-assisted productivity tools, and other approved software to support outreach and documentation. Coordination Coordinate outreach activities with designated ATCPHD grant personnel. Receive project assignments from designated grant staff while maintaining communication regarding outreach activities and deliverables. Participate in meetings and assist with meeting preparation and note-taking as assigned. Minimum Qualifications High school diploma or GED preferred. Community Health Worker certification preferred but not required Reliable transportation and valid driver's license. Strong communication, organization, and public speaking skills. Ability to work independently while collaborating effectively with ATCPHD staff and community partners. Ability to work occasional evenings and weekends based on community outreach needs. Contractor Expectations This Scope of Services defines the professional services expected of the contracted Community Health Worker. The CHW coordinates activities with ATCPHD personnel to support grant-funded initiatives while maintaining the independent contractor relationship established in the service agreement.
