Full Job Description Job Summary :

The Registered Dietitian (RD) is responsible for providing nutrition care and counseling to patients enrolled in the program. This role involves assessing, planning, and implementing individualized nutrition care plans to promote optimal health and manage chronic conditions. The RD must ensure that all services comply with Title 22 regulations, as well as other relevant state and federal laws.

Key Responsibilities:

Nutrition Assessment and Care Planning :

Conduct comprehensive nutrition assessments for all patients, including evaluating dietary needs, preferences, and medical conditions that may affect nutritional status. Develop and implement individualized nutrition care plans that are consistent with Title 22 regulations and tailored to each patient's health condition, treatment goals, and dietary preferences. Ensure that nutrition care plans are reviewed and updated regularly to reflect changes in the patient's health or treatment.

Patient Counseling and Education :

Provide one-on-one counseling to patients and their families regarding healthy eating habits, nutritional needs, and special dietary considerations (e.g., diabetes, hypertension, renal disease). Educate patients and caregivers on meal planning, portion control, and how to manage conditions through dietary modifications. Lead group nutrition education sessions when appropriate, focusing on general wellness and disease prevention. Collaboration with

Healthcare Team :

Collaborate with physicians, nurses, social workers, and other healthcare professionals to integrate nutrition care into the patient's overall care plan. Participate in interdisciplinary care team meetings to discuss patient progress, adjustments to nutrition plans, and any other health concerns related to nutrition. Provide consultation to staff regarding patients with special dietary needs or conditions that require nutritional modifications.

Menu Planning and Dietary Management :

Assist in the development and review of the ADHC's menu offerings to ensure they meet the nutritional requirements of patients, including those with specific health conditions (e.g., low sodium, low sugar, gluten-free). Work with food service staff to ensure that meals are prepared in accordance with the nutrition care plans, dietary restrictions, and patient preferences. Ensure compliance with Title 22's nutritional requirements for ADHC facilities, including meal patterns and serving sizes.

Monitoring and Documentation :

Monitor patients' progress and make adjustments to their nutrition care plans as needed, based on their clinical condition, lab results, and feedback from other healthcare providers. Keep accurate and up-to-date records of all nutrition assessments, care plans, education sessions, and other relevant patient information. Document changes in patient conditions and nutrition-related interventions in patient charts, ensuring that all records meet Title 22 and other regulatory standards. Compliance with

Regulations :

Ensure all nutrition-related services comply with Title 22 of the California Code of Regulations, as well as other applicable federal and state laws, including the Food and Drug Administration (FDA) and California Department of Public Health (CDPH) standards. Maintain knowledge of current nutrition guidelines, food safety standards, and dietary recommendations to ensure adherence to best practices in clinical nutrition.

Quality Assurance and Improvement :

Participate in quality assurance programs related to nutrition services, helping to identify areas for improvement and implementing strategies to enhance patient care. Assist in monitoring patient satisfaction regarding meal offerings and nutrition services, and work with food service teams to address any issues. Conduct audits of patient nutrition care and make recommendations for improvements in compliance with regulatory standards.

Staff Education and Training :

Provide nutrition-related training to facility staff, including caregivers and other healthcare providers, to improve overall nutrition care delivery. Stay current on nutrition trends, research, and best practices and share relevant information with the team to enhance patient care and dietary management.

Qualifications:

Education :

Bachelor's degree in Dietetics, Nutrition, or a related field from an accredited program. Completion of an accredited dietetic internship program.

Licensure :

Current and valid Registered Dietitian (RD) credential by the Commission on Dietetic Registration (CDR). Licensed by the California State Board of Dietetics (if applicable).

Experience :

At least 1 year of experience as a Registered Dietitian, preferably in an ADHC, long-term care, or other healthcare setting. Experience with nutrition care planning, patient education, and managing special dietary needs.

Skills :

In-depth knowledge of nutrition science, clinical nutrition, and food service management. Strong communication and interpersonal skills, with the ability to educate and collaborate with patients, families, and multidisciplinary healthcare teams. Ability to develop individualized care plans and adapt nutrition strategies to meet the specific needs of diverse patient populations. Excellent organizational and documentation skills, with the ability to comply with regulatory standards. Certifications (preferred): Certification in Diabetes Education (CDE) or other specialty certifications in clinical nutrition. Membership in the Academy of Nutrition and Dietetics (AND) or similar professional organization.

Pay:

From $42.00 per hour Expected hours: 10.0 per week
