In this vital role, you will assess individual nutritional needs, develop tailored care plans, and provide expert guidance to support optimal health outcomes. Your expertise will help manage chronic diseases, facilitate medical nutrition therapy, and promote healthy lifestyle changes across diverse patient populations. This position offers an exciting opportunity to collaborate with multidisciplinary healthcare professionals and make a meaningful difference in patients' lives through evidence-based nutrition interventions. Duties Conduct comprehensive patient assessments, including nutritional analysis and diagnostic evaluation to identify specific dietary needs. Develop and implement individualized care plans that incorporate medical nutrition therapy (MNT), weight management strategies, and chronic disease management protocols. Provide nutrition counseling using motivational interviewing techniques to motivate patients toward sustainable lifestyle changes. Manage specialized nutrition support procedures such as tube feeding and other clinical nutrition interventions. Collaborate closely with physicians, nurses, dietetic technicians, and other healthcare professionals to ensure integrated patient care. Document all assessments, care plans, and progress notes accurately within electronic health records (EHR) systems while maintaining compliance with healthcare regulations. Supervise and mentor junior staff or interns in dietary practices, ensuring adherence to clinical standards and quality care delivery. Qualifications Bachelor's degree or higher in Dietetics, Nutrition, or a related field; registration or licensure as a Registered Dietitian Nutritionist (RDN) is required. Proven experience in clinical nutrition, including hospital or long-term care settings, with expertise in medical nutrition therapy (MNT). Strong knowledge of physiology, disease processes, and dietary management for conditions such as geriatrics, weight loss, diabetes, and other chronic illnesses. Experience working with individuals with specific dietary needs and managing complex care plans. Proficiency in electronic health records (EHR) systems and EMR/EHR management for documentation and patient tracking. Excellent client communication skills with the ability to motivate change through motivational interviewing techniques. Supervising experience preferred, demonstrating leadership in dietary department operations or training staff members.
