Job Description

At the Lutheran Home at Southbury, we are dedicated to providing high-quality skilled nursing and rehabilitation services in a compassionate and resident-centered environment. We are seeking a compassionate and knowledgeable Part-Time Registered Dietitian to join our interdisciplinary care team. The Registered Dietitian will be responsible for assessing residents' nutritional needs, developing individualized nutrition care plans, and ensuring compliance with federal, state, and facility regulations. This role plays a critical part in supporting resident health, wellness, and quality of life within our skilled nursing facility. Key Responsibilities Complete nutritional assessments for new admissions, quarterly reviews, annual assessments, and residents identified as at risk. Develop, implement, and monitor individualized nutrition care plans. Evaluate residents for nutritional risk factors, weight changes, wounds, swallowing concerns, and other diet-related conditions. Collaborate with physicians, nursing staff, speech therapists, and other interdisciplinary team members to support resident care. Monitor and document resident nutritional status in accordance with regulatory requirements. Review and approve menus to ensure nutritional adequacy and compliance with state and federal guidelines. Provide nutrition education and counseling to residents, families, and staff as appropriate. Participate in care plan meetings and quality assurance initiatives. Ensure compliance with CMS, state, and facility nutrition and dietary regulations. Support the Dietary Services team with nutrition-related guidance and recommendations. Qualifications Registered Dietitian (RD/RDN) credential required. Current state licensure/certification as required. Bachelor's degree in Dietetics, Nutrition, or related field. Minimum of one (1) year of clinical nutrition experience preferred. Experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting preferred. Knowledge of CMS regulations and long-term care nutrition standards. Strong communication, organizational, and documentation skills. Ability to work independently and collaboratively with interdisciplinary teams.
