We are seeking a dedicated and highly skilled Registered/Licensed Dietitian to join our healthcare team. The ideal candidate will possess a comprehensive understanding of clinical nutrition, medical nutrition therapy, and patient-centered care. This role offers an opportunity to make a meaningful impact by developing personalized nutrition plans, collaborating with multidisciplinary healthcare professionals, and supporting patients with diverse dietary needs across various settings. The successful applicant will demonstrate expertise in nutrition counseling, chronic disease management, and the use of electronic health records to ensure optimal patient outcomes. The position collaborates closely with physicians and interdisciplinary care teams to ensure nutrition recommendations are integrated into coordinated patient care. Provide high-quality, person-centered Medical Nutrition Therapy (MNT), nutrition counseling, and education for patients with chronic and complex conditions including diabetes, cardiovascular disease, hypertension, hyperlipidemia, obesity/weight management, renal disease, gastrointestinal disorders, PCOS, eating disorders/disordered eating, wound healing, cancer nutrition, and other nutrition-related conditions Complete comprehensive nutrition assessments to evaluate nutritional status, identify nutrition-related problems, determine nutritional risk levels, and develop individualized nutrition care plans based on patient needs, preferences, and health goals Develop and implement evidence-based nutrition interventions, including diagnosis-specific nutrition recommendations and individualized nutrition prescriptions to support sustainable behavior change and improved health outcomes Counsel patients on nutrition strategies to improve disease management, including blood glucose management, weight management, cardiovascular health, and overall wellness Educate individuals on evidence-based nutrition principles, healthy eating habits, disease-specific nutrition therapy, and strategies to support long-term lifestyle changes Utilize motivational interviewing and patient-centered counseling techniques to support patient engagement, self-management, and adherence to nutrition recommendations Interpret and evaluate clinical data obtained from medical records, laboratory values, medications, and health history to guide nutrition interventions and recommendations Collaborate with physicians, care teams, and other healthcare professionals regarding nutrition recommendations, diet prescriptions, clinical concerns, and implementation of coordinated care plans Identify and prioritize patients requiring nutrition intervention and education based on referral needs, nutrition risk assessments, and clinical indicators Follow up with patients to monitor progress, provide ongoing support, reassess nutrition goals, and promote continued motivation and success within their nutrition program Maintain accurate, timely, and thorough documentation of all patient interactions, assessments, interventions, care plans, and outcomes within the electronic health record (EHR) Utilize virtual care platforms and digital tools to provide effective nutrition counseling and education to patients Assist in the development, design, review, and improvement of patient education materials, nutrition resources, presentations, webinars, and outreach initiatives Participate in quality improvement initiatives, program development, and innovation efforts to improve nutrition care delivery and patient outcomes Stay current with nutrition research, clinical guidelines, and evidence-based practices through continuing education and professional development Participate in team meetings, cross-functional initiatives, and special projects as assigned Support all UM and UMACO initiatives Use, protect, and disclose patient's protected health information (PHI) in accordance with Health Portability and Accountability Act (HIPAA) standards Follow all United Medical policies, uphold professional standards, and perform work responsibilities in a manner that is respectful to others Maintain consistent, reliable and regular in-office attendance to foster a productive and collaborative work environment, enhance teamwork, ensure operational efficiency, and support client interactions Perform other duties or projects as needed Required Education and Experience Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR) and in good standing. Current Delaware Registered Dietitian licensure (or ability to obtain prior to employment). Bachelor's degree or higher in Nutrition, Dietetics, or a related field. Candidates with a Master's degree in Nutrition, Dietetics, or a related field preferred. Candidates who achieved RD/RDN credential prior to the January 1, 2024 master's degree requirement are eligible with a bachelor's degree. Preferred Education and Experience Two or more years of clinical nutrition experience providing Medical Nutrition Therapy. Experience counseling patients with chronic disease management, including diabetes, weight management, cardiovascular disease, renal disease, gastrointestinal disorders, and other complex medical conditions. Experience with bariatric nutrition and procedures including: Gastric bypass, Sleeve Gastrectomy, Laparoscopic adjustable gastric banding, Non-surgical gastric balloon. Position Type / Expected Hours of Work This is a full-time position working 40 hours per week, Monday through Friday. The business office is open from 8:00 AM - 6:00 PM. This position is not remote.
