Job Description

Registered Dietitian Job Description Registered Dietitian maintains provision of optimum nutritional care and education for the patients and their families. Coordinates patient nutrition services by working closely with the patient and the patient's family and other members of the health care team in a continuum of care from the acute care to the home health setting.

Required:
BS Degree, Licensed Dietitian Nutritionist (LDN), Food Handers Certificate within 30 days of hire
Pay:
based on experience, starting at $26.35/hour Responsibilities Collaborates with other hospital services to plan and implement patient care and nutritional education, as necessary., Consults with all disciplines and medical staff regarding patient nutritional or discharge planning needs. Attends and participates in interdisciplinary rounds and meetings acting as a liaison/consultant on nutritional practice., Counsels patients and/or family on prescribed diet regime that is both age appropriate and culturally relevant in a manner that facilitates understanding, documenting the findings in the patient's medical record., Identifies nutritional deficiencies or special nutritional needs by nutritionally screening patients for identifiable risk factors., Knowledge of DFM, call center software., Maintains pertinent patient data necessary to recommend, prescribe, or modify therapeutic diets as needed to meet the nutritional needs of the patients. Monitors the nutritional status of patients and documents pertinent nutritional information in the medical record., Monitors and reevaluates the patient's response and tolerance to the established nutritional care plan., Performs comprehensive nutritional assessments, nutrition diagnosis, interventions, and monitors and evaluates patient outcomes, when appropriate. Develops nutritional care plans., Plans, implements, and presents pertinent educational programs, serving as a resource to staff, other health care professionals, or the community., Prepares and calculates therapeutic diets and/or formulas. Assists in the development, planning, execution, and approval of patient menus. Requirements Bachelor's Degree (Required)Food Handlers Certificate (within 30 days of hire)
