NUTRITIONIST - HOME CARE
- Per Diem Show Map Location 333 Earle Ovington Boulevard, Suite 600, Uniondale, NY, 11553, United States Job Category Clinical Employee Type Per Diem Minimum Experience 1 Year Description The Nutritionist provides Home Care visits to patients with nutritional needs and serves as a resource to staff on specific patient nutritional issues related to the overall medical plan of care. Supports Healthcare System's mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.
JOB DUTIES / RESPONSIBILITIES
Makes home visits to conduct initial and ongoing assessments of patient/caretaker nutritional needs. Implements nutritional care plan, complying with the medical diagnosis needs and physician orders. Assesses patients who have difficulty in complying with dietary restrictions, difficulty in understanding complex dietary restrictions, exhibit high nutritional risk, show evidence of malnutrition or slow wound healing process, and other nutritional problems for which nursing staff desire additional consultation. Establishes short and long-term health goals with patients. Develops nutrition & exercise plans while taking into account the client's culture, budget, tastes, abilities. Evaluates if the meal plans have a positive effect on patients' health. Creates educational material about healthy eating habits serves as a consultant to appropriate staff to provide education regarding nutrition and healthcare when requested. Records observation and activities related to patient intervention, finding care outcomes and plans in accordance with agency policy. Documents patient progress in timely manner following the home visit. Assists with the coordination of the total care plan and maintains continuity of patient care by liaising and cooperating with other health care professionals assigned to the same patient. Makes recommendations to the health care team regarding patient needs and rehabilitation potential. Participates in quality improvement activities. Attends quality improvement committee meetings as requested.
