Optimum Care Partners is seeking a compassionate, motivated, and patient-focused Registered Dietitian (RD/RDN) to join our primary care team. This role will focus on helping patients achieve sustainable weight loss, improve overall health, and manage chronic conditions through personalized nutrition counseling and lifestyle coaching. As part of our multidisciplinary team, you will work alongside physicians and advanced practice providers to develop individualized treatment plans that empower patients to make lasting lifestyle changes.
Responsibilities:
Provide individualized nutrition assessments and counseling for weight management. Develop customized meal plans based on each patient's health goals, medical history, and lifestyle. Educate patients on healthy eating habits, portion control, behavior modification, and long-term weight maintenance. Support patients receiving weight loss medications by providing ongoing nutritional guidance and monitoring. Collaborate with providers to create comprehensive treatment plans. Monitor patient progress and adjust nutrition plans as needed. Document patient encounters accurately in the electronic medical record (EMR). Educate patients on nutrition related to diabetes, hypertension, high cholesterol, and other chronic diseases. Encourage accountability and motivation through regular follow-up visits.
Qualifications:
Registered Dietitian (RD/RDN) credential required. Licensed Dietitian in the State of Oklahoma (or eligible for licensure). Bachelor's degree in Nutrition, Dietetics, or related field (Master's preferred). Experience in weight management, obesity medicine, primary care, or lifestyle medicine preferred. Strong communication and motivational interviewing skills. Ability to build trusting relationships with patients from diverse backgrounds.
Preferred Experience:
Medical weight loss programs GLP-1 medication support (Semaglutide, Tirzepatide, etc.) Diabetes education Nutrition counseling Behavior change coaching Group education classes (preferred but not required)
