ROLE AND RESPONSIBILITIES

The Assistant Director of Nursing (ADON) - Renal Clinic Services supports the Director of Nursing and interdisciplinary care team in the coordination, oversight, and delivery of high-quality renal care services within the Skilled Nursing Facility (SNF). This role assists with clinical operations, regulatory compliance, quality improvement initiatives, staff development, and patient care management for residents receiving nephrology-related services. The ADON serves as a clinical resource to nursing staff and helps ensure safe, effective, and resident-centered care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Clinical Leadership & Patient Care Assist in coordinating and overseeing renal clinic operations within the SNF setting. Support the implementation of individualized care plans for residents with chronic kidney disease (CKD), end-stage renal disease (ESRD), dialysis needs, and related conditions. Collaborate with nephrologists, advanced practice providers, nursing staff, dietitians, social workers, and other interdisciplinary team members to optimize resident outcomes. Monitor resident clinical status and assist with the management of renal-related complications and chronic conditions. Promote evidence-based practices and adherence to established clinical protocols. Quality & Regulatory Compliance Assist in maintaining compliance with federal, state, and local regulations, as well as facility policies and procedures. Participate in quality assurance and performance improvement (QAPI) activities related to renal services. Support preparation for surveys, audits, and accreditation reviews. Monitor clinical documentation to ensure accuracy, completeness, and regulatory compliance. Assist with tracking and reporting quality metrics and clinical outcomes. Staff Development & Supervision Provide clinical guidance, education, and support to nursing staff caring for residents with renal conditions. Participate in staff orientation, training, competency validation, and ongoing professional development. Assist with scheduling, staffing coordination, and performance management activities as assigned. Foster a culture of teamwork, accountability, and continuous improvement. Care Coordination & Communication Facilitate communication among residents, families, providers, dialysis centers, and facility staff. Assist in coordinating specialty appointments, treatments, laboratory monitoring, and follow-up care. Participate in interdisciplinary meetings and care conferences. Support transitions of care and continuity of services across care settings. Administrative Responsibilities Assist the Director of Nursing in daily operational oversight of renal clinic services. Maintain records, reports, and clinical data as required. Participate in policy and procedure development and implementation. Support inventory management and appropriate utilization of clinical resources and supplies.

QUALIFICATIONS

Education Graduate of an accredited school of nursing. Licensure Current, active Registered Nurse (RN) license in the state of practice. Current CPR/BLS certification required. Experience Minimum of 3 years of nursing experience, preferably in skilled nursing, long-term care, nephrology, dialysis, or chronic disease management. Prior leadership, charge nurse, or supervisory experience preferred. Knowledge, Skills, and Abilities Knowledge of renal disease management, dialysis principles, and geriatric nursing care. Understanding of SNF regulations, quality standards, and clinical documentation requirements. Strong organizational, communication, and problem-solving skills. Ability to work collaboratively with interdisciplinary teams. Proficiency with electronic health records and clinical software systems. Physical Requirements Ability to stand, walk, bend, lift, and assist residents as needed. Ability to perform essential nursing functions in a clinical environment. Ability to travel between facility locations if required. Work Environment Skilled Nursing Facility and renal clinic environment. Frequent interaction with residents, families, providers, and interdisciplinary healthcare professionals. May require occasional evening, weekend, or on-call responsibilities based on operational needs.
