$115,000 to $130,000 per year + bonus pay for performance metrics About Forest Ridge Forest Ridge is a 4 star, 80-bed, all-private-room skilled nursing and rehabilitation community located on the hospital campus in Woodland Park, Colorado. We serve long-term care, post-acute skilled, and memory care residents, and we hold a VA contract to provide rehabilitation, long-term care, and respite services for veterans. We are located in a mountain community, which means two things: our team knows every resident by name, and our leaders are genuinely part of the day-to-day, not sitting behind a closed door. Our nursing turnover runs below the Colorado state average, and we intend to keep it that way. The Role We are looking for a Director of Nursing who leads with two hands on the wheel: clinical compliance and people. This is a hands-on leadership position with an 80-bed census with three distinct populations. You will own survey readiness and regulatory compliance for the building, and you will be the person your nurses, CNAs, and fellow department heads trust and want to work for. You will report to the Administrator and serve on the facility leadership team alongside Therapy, Social Services, Dietary, Activities, and Business Office leadership. What You'll Do Regulatory Compliance & Survey Readiness Own continuous survey readiness for annual, complaint, and infection-control surveys under CMS Requirements of Participation and Colorado Department of Public Health and Environment regulations Direct the QAPI program, lead the QAA committee, identify trends from clinical data, and drive corrective action to closure Manage the plan of correction process from entrance conference through credible allegation of compliance Oversee infection prevention and control, antibiotic stewardship, and NHSN reporting Maintain accurate clinical documentation standards, MDS accuracy, care plan compliance, and PDPM support Ensure compliance with abuse prevention, reporting, and investigation requirements Clinical Leadership Direct all nursing operations across long-term care, skilled/post-acute, and memory care Oversee admissions from a clinical standpoint, evaluating referrals for appropriateness and ensuring the building can safely meet each resident's needs Lead clinical response to changes in condition, reduce avoidable hospital readmissions, and support physician and NP collaboration Guide dementia-specific care practices, behavior management, and person-centered approaches in memory care Manage nursing budget, staffing ratios, supply utilization, and agency spend Team Building & Staff Development Recruit, hire, onboard, mentor, and retain a stable nursing team. Turnover reduction is a core measure of success in this role Develop the ADON, unit managers, and charge nurses into stronger clinical leaders Direct orientation, annual competencies, and ongoing in-service education Handle performance management fairly and consistently, with documentation that holds up Build a floor culture where staff raise concerns early and feel supported when they do Partner across departments so nursing is a collaborator, not a silo What We're Looking For Required Current, unencumbered Colorado RN license (or compact license with eligibility to practice in Colorado) 3+ years of nursing leadership experience in a skilled nursing or long-term care setting (DON, ADON, or nurse manager) Working knowledge of CMS Requirements of Participation, F-tags, the survey process, and plan of correction development Demonstrated experience with QAPI, infection control, and clinical quality measures Current CPR/BLS certification Track record of building and retaining a nursing team Strongly Preferred Prior Director of Nursing experience in a skilled nursing facility History of successful state surveys with minimal deficiencies Memory care or dementia-specific programming experience Familiarity with PointClickCare BSN Who Thrives Here You walk the halls. You hold a high bar and people still want to work for you. You collaborate well with an Administrator and other department heads rather than protecting turf. You are comfortable being the clinical decision-maker in a building where you are the top nurse. Schedule Full-time, Monday through Friday, with participation in the facility's administrative on-call rotation and availability for after-hours clinical escalation.
Compensation & Benefits Salary:
