Full Job Description Job Summary Position:
Home health and Hospice RN Director of Nursing Reports to:
Administrator Job Summary:
The Clinical Director is responsible highest level of compliance and patient care, responsibilities include, but are not limited to, ensure patient care coordination, management, clinical oversight, implementation and ongoing assessment of the Hospice Plan of Care for patients, Order review and approval, Patient advocacy, participation in case conferences(s), assists Branch director with addressing recertification or discharge decisions as necessary, production of reports, assistance with Quality improvement initiatives, and compliance with Federal, State, and Local regulations. Maintains the quality of patient care in accordance with the Branch's objectives and policies, through planning, coordination and evaluation of the hospice services provided.
Essential Duties and Responsibilities:
Leadership Lead assigned team members. Facilitate growth within assigned team. Assist with the development and retention of "A" players within assigned team. Oversee and appropriately assign work to assigned team members based upon team member roles, responsibilities and competencies. Assist in facilitating empathetic accountability of staff, inclusive of assisting with performance evaluations on staff in accordance with organizational policies and procedures. Attentive follow-up with staff to ensure assigned goals are being met in accordance with Federal, State and Local regulations. Sets standard for assigned team members by displaying organizational values, work ethic, persistence, follow-through, listening skills, oral/written communication skills, integrity, honesty and follow-through of goals and objectives. Ensures efficiency of execution of organization mission, vision, values and strategies within assigned team. Growth Assists to responsibly drive patient census in accordance with organization mission, vision, values and strategies, as well as regulatory requirements. Assist with the development of assigned team members to encourage their continued growth within the Company. Continually work closely with exterior team members, inclusive of sales and marketing staff members, to develop new beneficial relationships as well as maintain and strengthen existing relationships. Quality Participate in high value QAPI program and perform required patient chart audits. Lead patient care conferences with internal staff and field staff. Ensure OASIS accuracy and appropriate Plan of Care development. Reviews and approves applicable patient Orders. Treats patients, as necessary, based upon independent review of patient needs and in accordance with the patient's Plan of Care. Responsible for coordination between patients, physicians and applicable field staff members. Assists in leading IDG meetings. Assists with ensure of compliance with all Federal, State, Local and accreditation body requirements and assumes responsibility to oversee that the same are followed by all clinical staff. quality, interdisciplinary approach to hospice services. In addition to the above-mentioned, core duties, Clinical Manager shall perform all other duties as required and/or requested. Physical Demands and Work Environment:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. The employee frequently is required to use hands or fingers, handle, or feel objects, tools or controls. The employee must occasionally lift, push, pull, carry and/or move up to 30 pounds and may occasionally need to move a patient weight up to 250 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus. The noise level in the work environment is usually moderate. Position may be stressful. The ability to access patients' homes which may not be routinely wheelchair accessible. Travel may be required to and from patient's home when supervision of care is scheduled. Incumbent will operate in an office setting, and be required to use a computer with keyboard, telephone or handheld mobile device for extended periods of time, and office machinery as needed. Hearing, eyesight and physical dexterity must be sufficient to perform supervisory visit, a physical assessment of the patient's condition and to perform patient care when necessary. Education, Certification(s), and/or License(s)
Required:
Must be a graduate of an accredited School of Nursing Must be a Registered Nurse licensed by the State. Must have a minimum of 2 years nursing experience in Home Health, Hospice or Skilled Home Care (Hospice preferred), with: At least one (1) year of supervisory experience, inclusive of team leadership experience.
Other Requirements:
At least one (1) year of supervisory experience. Ability to market and deal tactfully with customers and community. Knowledge of business management, governmental regulations and CHAP standards. Has access to all patient medical records and patient financial accounts as well as personnel records of department staff and thus must be knowledgeable on HIPAA regulations. Knowledge of Microsoft Office 365 and Agency's EMR system (Devero) preferred.
Pay:
$86,785.11 - $104,515.40 per year
