Responsibilities Provide expert guidance on wound care management, including assessment, treatment planning, and documentation review to optimize resident outcomes. Supervise and support nursing staff in delivering high-quality patient care aligned with best practices and regulatory standards. Collaborate with interdisciplinary teams to develop and implement individualized care plans that address residents' specific needs, including those with memory care or complex medical conditions. Ensure compliance with long-term care regulations, Medicare requirements, HIPAA privacy rules, and documentation protocols within EMR/EHR systems. Oversee staff scheduling, shift management, and training initiatives to promote a cohesive, efficient care environment. Conduct regular audits of medical records and clinical documentation to maintain accuracy and regulatory adherence. Lead quality improvement projects focused on wound healing processes, patient safety, and resident satisfaction to foster continuous improvement. Provide education to nursing staff on wound care, skin assesment. Coordinate wound rounds and follow ups with physicians. Conduct weekly wound rounds and maintain an accurate wound tracking list. Qualifications Proven nursing leadership experience within long-term care facilities or assisted living environments, with a strong background in wound care management. Current NY State Registered Nurse License. Extensive knowledge of nursing administration, medical terminology, physiology, anatomy, and regulatory compliance standards such as HIPAA and Medicare regulations. Demonstrated supervising experience managing care facility teams in fast-paced settings like home health agencies or hospice care programs. Familiarity with EMR/EHR systems for documentation review and medical records management. Strong understanding of resident-centered care principles across diverse populations including memory care residents. Excellent communication skills coupled with the ability to lead clinical teams effectively while maintaining high standards of patient care. Ability to manage multiple priorities efficiently while ensuring compliance with all long-term care regulations and safety protocols. $110,000.00 - $135,000.00 per year
