Resident Care Manager (RCM) Forest Grove Post Acute Position Summary The Resident Care Manager (RCM) serves as a key member of the interdisciplinary team and is responsible for coordinating resident care, monitoring clinical needs, and supporting effective communication between departments to ensure high-quality outcomes. The RCM works closely with nursing, therapy, social services, dietary, activities, admissions, and facility leadership to promote resident-centered care, identify opportunities for improvement, and ensure residents receive appropriate services throughout their stay. This role requires strong clinical knowledge, organization, attention to detail, and the ability to collaborate effectively across departments. Essential Duties and ResponsibilitiesResident Care Coordination Monitor resident care needs and collaborate with the interdisciplinary team to ensure appropriate services are provided. Participate in daily clinical meetings and communicate resident updates, concerns, and changes in condition. Assist with identifying barriers to successful resident outcomes and develop solutions with the care team. Support effective communication between nursing, therapy, providers, families, and other departments. Participate in care plan meetings and ensure resident needs, preferences, and goals are addressed. Assist with coordinating resident transitions, including admissions, transfers, and discharges. Clinical Documentation & Quality Support Review resident records to ensure documentation accurately reflects resident needs, services provided, and outcomes. Collaborate with nursing leadership and clinical team members to identify documentation opportunities. Support completion of required assessments, care planning, and clinical documentation processes. Participate in quality improvement initiatives and assist with monitoring facility goals. Assist with survey readiness activities and regulatory compliance efforts. Resident Monitoring & Follow-Up Maintain awareness of resident status and changes in condition. Follow up on clinical concerns, care needs, and interdisciplinary recommendations. Promote timely communication regarding changes in resident condition. Assist in ensuring physician, family, and responsible party communication occurs as appropriate. Support implementation of individualized resident care plans. Team Collaboration & Leadership Serve as a clinical resource and support for nursing and interdisciplinary team members. Promote teamwork, accountability, and a positive resident-centered culture. Collaborate with department leaders to address concerns and improve resident outcomes. Participate in facility meetings, committees, and quality initiatives as assigned. Demonstrate professionalism, ownership, and commitment to Forest Grove Post Acute's mission and values.
Qualifications Required:
Experience working in a skilled nursing facility environment. Strong communication, organization, and critical thinking skills. Ability to prioritize multiple responsibilities in a fast-paced healthcare setting. Commitment to providing high-quality, resident-centered care.
Preferred:
Licensed Nurse (LPN/RN preferred). Previous experience in skilled nursing clinical coordination, MDS, care management, or leadership. Knowledge of SNF regulations and clinical processes.
Pay:
$46.00 - $53.00 per hour
