Job Description

Overview We are seeking a highly skilled and dedicated SNF Rehabilitation Director to lead our rehabilitation services within a skilled nursing facility. The ideal candidate will possess extensive expertise in rehabilitation medicine, clinical team leadership, and patient care management. This pivotal role involves overseeing all aspects of rehabilitation program development, ensuring compliance with regulations, and fostering a collaborative environment that promotes optimal patient outcomes. The Rehabilitation Director will be instrumental in coordinating multidisciplinary teams to deliver comprehensive rehabilitative services tailored to individual needs, including those with disabilities or visual impairments. This position offers an opportunity to make a meaningful impact on patient recovery and quality of life through innovative and evidence-based practices. Responsibilities Lead and manage the rehabilitation department, including physiotherapy, occupational therapy, and speech-language pathology teams, ensuring high-quality patient care. Develop, implement, and monitor rehabilitation programs aligned with current regulations and best practices in long-term care and skilled nursing facilities. Supervise clinical staff, providing mentorship, training, and performance evaluations to foster professional growth and team cohesion. Ensure compliance with all federal, state, and local rehabilitation regulations while maintaining accreditation standards. Oversee patient assessments, treatment planning, and progress documentation to optimize recovery outcomes. Coordinate with interdisciplinary teams to tailor rehabilitation plans for patients with diverse needs, including those with disabilities or visual impairments. Manage resources effectively by overseeing equipment use, manual handling protocols, and safety procedures such as basic life support (BLS) and cardiopulmonary resuscitation (CPR). Promote a culture of continuous improvement by integrating physiologic knowledge of anatomy and physiology into clinical practices. Facilitate staff training on heavy lifting techniques, manual handling safety, and rehabilitation regulations to ensure a safe working environment. Advocate for patient-centered care that emphasizes rehabilitation services management, patient safety, and functional independence. Qualifications Extensive experience in skilled nursing facility or rehabilitation center settings with a focus on long-term care environments. Proven leadership experience managing multidisciplinary clinical teams within a rehabilitation services context. Strong knowledge of rehabilitation medicine principles, physiology, anatomy, and relevant regulations governing rehabilitation services. Certification in CPR/BLS is required; additional certifications in acute care or specialized therapy fields are advantageous. Demonstrated ability to develop comprehensive patient care plans that address individual needs across diverse populations including those with disabilities or visual impairments. Familiarity with heavy lifting protocols, manual handling techniques, and safety standards essential for patient mobility assistance. Excellent communication skills to collaborate effectively with staff members, patients, families, and external healthcare providers. Prior experience working with individuals requiring complex rehabilitative interventions is highly desirable.
