Are you a rehabilitation leader with Skilled Nursing Facility (SNF) and Long-Term Care (LTC) experience who is passionate about improving resident outcomes and building strong therapy teams? We are seeking an experienced Director of Rehabilitation (DOR) to lead our therapy department with clinical excellence, compassion, and accountability. In this role, you will oversee rehabilitation services within a SNF/LTC environment , support and develop a high-performing interdisciplinary therapy team, improve resident outcomes, and ensure quality care while maintaining compliance with regulatory and facility standards. Job Summary We are seeking an experienced and passionate Director of Rehabilitation (DOR) to oversee rehabilitation services in a skilled nursing and long-term care setting. The DOR will be responsible for the overall coordination and leadership of therapy operations, ensuring residents receive high-quality, individualized rehabilitation services. The ideal candidate will have strong SNF/LTC experience , understand the unique clinical and operational needs of the post-acute population, and be able to effectively lead a multidisciplinary therapy team.
This role will focus on:
Leading and overseeing daily rehabilitation operations within the SNF/LTC setting Developing and supporting effective therapy programs based on resident needs Promoting strong clinical outcomes, resident participation, and quality of life Collaborating with nursing, social services, dietary, activities, and other departments as part of the interdisciplinary team Supporting therapists in delivering individualized, resident-centered care Monitoring therapy productivity, staffing, documentation, and overall department performance Ensuring compliance with CMS, state, and other applicable SNF/LTC regulations Supporting care plan meetings, clinical documentation, and survey readiness Building positive relationships with residents, families, physicians, and facility leadership Mentoring, coaching, and developing therapy staff Identifying opportunities to improve processes, communication, and resident outcomes Responsibilities Lead and oversee daily operations of the rehabilitation department. Supervise and support therapy professionals, including PT, OT, SLP, assistants, aides, and students. Manage staffing, scheduling, productivity, and department performance. Ensure high-quality patient care while maintaining compliance with regulatory standards. Develop and implement effective rehabilitation programs based on patient needs. Monitor clinical outcomes, documentation, and quality measures. Mentor and support team members through training and professional development. Collaborate with facility leadership and interdisciplinary teams to promote positive resident outcomes. Identify opportunities for program growth and continuous improvement. Experience Licensed Physical Therapist, Occupational Therapist, Speech-Language Pathologist, or applicable therapy discipline. Previous rehabilitation leadership or management experience preferred. Experience within a skilled nursing, long-term care, or rehabilitation setting preferred. Strong understanding of therapy operations, clinical documentation, and regulatory requirements. Proven ability to lead, motivate, and develop a multidisciplinary team. Excellent communication, organization, and problem-solving skills. Current CPR/BLS certification as required.
