Job Summary:

The Cardiac Rehab Exercise Physiologist is responsible for managing individualized patient care by promoting and restoring patient's health through the cardiac rehab process. Develops individualized exercise prescriptions and oversees exercise progression for patients with heart disease and other chronic diseases. Collaborates with a multi-disciplinary team to empower patients recovering from heart events to achieve total wellness.

Duties/Responsibilities:

Identifies patient care requirements by establishing personal rapport with potential and actual patients. Follows ACSM Guidelines for Exercise Testing and Prescriptions. Follows AACVPR and Medicare Guidelines for Cardiac Rehab. Determines if cardiac diagnosis meets acceptable criteria to attend ICR. Communicates with referring provider if diagnosis is not acceptable per insurance guidelines. Schedules patients for appropriate cardiac rehab appointments (ICR initial consultations and stress tests at SDC). Conducts 1:1 consultation to further inform patients and their families of cardiac rehab environment with review of heart event, CAD risk factors, medication compliance, and past medical history. Gathers current medication list and ensures patients fully understand reason for and actions of all cardiac medications. Gathers baseline conditioning/cardiac status and determines if initial cardiac rehab stress test is recommended. Schedule patient appropriately. Provides opportunities for questions & concerns to be addressed. Reviews ICR insurance coverage with patients obtained by SDC Authorizations team. Respects patient confidentiality and privacy by maintaining the security of all patient communications, per current HIPAA regulations. Monitors telemetry during cardiac rehab exercise classes with basic ECG interpretation knowledge. Establishes a compassionate environment by providing emotional and psychological support in a professional manner to patients and families. Contacts physician if indicated to modify medical treatment plan Places appropriate orders in EMR system per Provider orders Assures quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals, and making or recommending necessary adjustments Evaluates physical complaints by thoroughly questioning and performing physical assessment Recognize need for interventions in evaluation of angina, hyper/hypotension, hypo/hyperglycemia, congestive heart failure, arrhythmias, and takes appropriate medical action Review cardiac rehab stress tests, lipids, echo results with currently enrolled patients Maintains a cooperative relationship among health care team members Emergency support when necessary Maintains professional and technical knowledge by attending educational workshops/webinars/seminars/conferences.

Strong analytical and problem-solving skills. Ability to function well in a high-paced and at times stressful environments. Basic computer knowledge with experience using Microsoft Office 365.

Education and Experience:

Exercise Physiologist:

Bachelor's Degree or Master's Degree in Exercise Science from an accredited University ACSM-EP or CEP certification preferred AHA-Basic Life Support (BLS) Certification; BLS and ACLS is preferred. 400 + hours of experience in Cardiac Rehab; Experience with Intensive Cardiac Rehab is preferred Familiar with AACVPR Guidelines for Cardiac Rehab Experience with

EPIC EMR

system and Scottcare/Versacare Tele system preferred

Physical Requirements:
