Job Overview We are seeking a compassionate and clinically skilled Mental Health Therapist to provide psychotherapy services to geriatric patients in skilled nursing facilities, assisted living communities, memory care units, rehabilitation centers, and other long-term care settings. The Mental Health Therapist will work as part of an interdisciplinary behavioral health team and collaborate closely with psychiatric providers, nursing staff, social services, facility leadership, residents, and families. This Mental Health Therapist position provides an opportunity to make a meaningful impact on the emotional well-being, behavioral health, and overall quality of life of older adults. Responsibilities Conduct comprehensive behavioral health and psychosocial assessments. Develop individualized treatment plans based on patient needs, clinical findings, and treatment goals. Provide individual psychotherapy to geriatric patients. Provide family and caregiver support and education. Diagnose and treat behavioral health conditions within the scope of licensure. Monitor patient progress and modify treatment plans as clinically indicated. Provide behavioral interventions for residents experiencing challenging behaviors. Provide crisis intervention and de-escalation when necessary. Collaborate with psychiatric nurse practitioners, supervising physicians, nursing staff, social workers, and other members of the care team. Participate in interdisciplinary care plan meetings as requested. Communicate effectively with facility staff and families regarding patient needs and treatment. Maintain accurate, timely, and compliant clinical documentation. Complete all required documentation within established timeframes using the electronic health record (EHR). Maintain established productivity standards. Comply with

HIPAA, CMS

regulations, state licensing requirements, and company policies. Travel between assigned facilities as needed. Qualifications Master's degree or higher in social work, psychology, counseling, or a related field from an accredited institution. Valid licensure or certification as a licensed mental health counselor (LMHC), licensed clinical social worker (LCSW), psychologist (PhD/PsyD), or equivalent credential. Demonstrated experience working with geriatric population. Proficiency in clinical counseling techniques such as cognitive behavioral therapy (CBT), psychodynamic therapy, trauma-focused therapy, and behavior management strategies. Strong knowledge of

ICD-9/ICD-10

coding systems for accurate diagnosis documentation. Experience delivering telehealth services using secure platforms while maintaining clinical ethical guidelines and HIPAA compliance. Ability to handle crisis situations with calmness and professionalism; experience in crisis intervention and crisis management is essential.
