Kaiser Permanente is seeking an RN Navigator - Diabetes/Endocrinology Educator to join our Southwood Specialties team in Jonesboro, GA. This is an opportunity to move beyond traditional bedside nursing and make a lasting impact by helping patients and families understand their diagnoses, navigate treatment options, access resources, and stay engaged throughout their healthcare journey. As an RN Navigator, you'll serve as a trusted connection between patients, families, physicians, and the broader healthcare team while helping patients receive the right care at the right time. What You'll Do Educate patients and families about diagnoses, treatment options, medications, and available resources. Assess patients, provide triage, manage symptoms, and coordinate individualized plans of care. Serve as a key point of contact between patients, physicians, families, and multidisciplinary healthcare teams. Coordinate appointments, consultations, procedures, referrals, diagnostic testing, and support services. Follow patients throughout the continuum of care and assist with transitions between outpatient, hospital, home, and community settings. Provide ongoing patient and family education throughout the treatment process. Support medication teaching, self-injection education, device programming/maintenance, and other specialty-specific education. Monitor treatment plans, patient compliance, symptoms, side effects, and clinical outcomes. Connect patients with appropriate community resources, support groups, and educational programs. Collaborate with physicians and healthcare partners to address barriers to care and facilitate timely interventions. Track outcomes, identify quality indicators, support process improvement, and participate in research or clinical initiatives. Maintain accurate documentation and patient records. Serve as a resource, preceptor, and mentor to other healthcare team members and nursing students.

What We're Looking For Required:

Associate degree in Nursing. Minimum 3 years of current RN clinical experience in the area of specialty. Current Georgia Registered Professional Nurse (RN) license at time of hire. Basic Life Support (BLS) certification at time of hire.

Preferred:

BSN degree. Experience as a Clinical Nurse Coordinator, Clinical Navigator, or Case Manager . Experience working with Diabetes and/or Endocrinology patients.
