We are seeking compassionate, highly motivated Clinical Health Coaches to provide ongoing support to patients living with chronic medical conditions. This is much more than a traditional Medical Assistant position. You will become a trusted member of each patient's healthcare team, helping them navigate their healthcare journey between office visits while working alongside physicians, nurses, behavioral health professionals, and care coordinators. What You'll Do As a Clinical Health Coach, you will: Develop trusted relationships with assigned patients. Conduct scheduled telephone and virtual patient outreach. Encourage patients to actively participate in their care plans. Review remote monitoring alerts and escalate concerns appropriately. Assist patients with chronic disease self-management. Reinforce physician treatment plans. Identify barriers to care, including transportation, food insecurity, medication access, and other social needs. Coordinate services with physicians, specialists, pharmacies, community resources, and caregivers. Document patient interactions accurately in our care management platform. Monitor patient engagement and help improve clinical outcomes. Communicate professionally with physician offices and interdisciplinary care teams. Required Qualifications Medical Assistant certification or equivalent clinical experience. Minimum of 2 years of patient care experience. Strong understanding of medical terminology. Experience communicating with patients by telephone and in person. Excellent customer service skills. Strong written documentation skills. Computer proficiency. Ability to manage multiple priorities while maintaining attention to detail. Pay:
$23.75 - $28.65 per hour Application Question(s): Have you worked in Primary Care, Internal Medicine, or Cardiology? Describe a difficult patient interaction and how you resolved it. Are you comfortable speaking with patients on the telephone for most of your day? Describe a time when you took initiative without being asked. How comfortable are you learning new software applications? Describe your experience documenting patient encounters in an electronic medical record. Have you participated in Chronic Care Management, Care Coordination, or Population Health? Why do you want to help patients with chronic medical conditions?
