Job Summary Health Educator is responsible for planning, implementing, coordinating, and evaluating evidence-based public health programs and educational initiatives that promote health, prevent disease, and improve community health outcomes. This position develops and implements grant-funded programs, conducts community outreach and health education, supports clinical team and collaborates with community partners to address health disparities and improve access to preventive services.

Health Educator supports public health policy and health promotion initiatives, ensures compliance with grant requirements and public health standards, and performs other duties as assigned in support of the mission of the health department. Required Education and Experience

• High school diploma or equivalent required.
• Bachelor's degree in public health, health sciences, human services, social work, or closely related field.
• Completion of National Incident Management System (NIMS) training within six (6) months of employment.
• Proficiency in Microsoft Office and common computer/database applications.
• Valid driver's license, proof of personal vehicle insurance, and ability to use personal transportation for work purposes.
• Ability to travel throughout the TRPHD seven-county service area and occasionally beyond.
• Demonstrated ability to work effectively with diverse populations and maintain cultural awareness and sensitivity.

Preferred qualifications:

, Health Promotion, Community Health, Health Education, or a closely related field from an accredited college or university. of progressively responsible experience in public health or community health education, including the planning, implementation, and evaluation of health promotion programs in community-based settings. Demonstrated experience coordinating public health programs or initiatives within a local, state, nonprofit, healthcare, or other public or private agency. Demonstrated experience developing, implementing, managing, and evaluating evidence-based public health programs, including monitoring outcomes and ensuring program compliance with applicable standards and grant requirements. Demonstrated experience collaborating with community partners and multidisciplinary teams to improve population health and reduce health disparities. Demonstrated experience in grant writing and the development, implementation, management, evaluation, and fiscal oversight of grant-funded programs. Duties and Responsibilities Community Outreach and Client Support Provide health education on immunizations, communicable disease prevention, oral health, nutrition, chronic disease prevention, substance use, and other public health topics. Assist with clinical and preventive health services, including but not limited to: chronic disease education, vaccination clinics, dental programs, pregnancy testing, STI education, and other public health initiatives. Connect individuals and families with healthcare providers, community resources, and social services; assist with referrals, appointment scheduling, and care coordination. Community Partnerships and Program Development Establish and maintain collaborative partnerships with healthcare providers, schools, long-term care facilities, community organizations, and other stakeholders. Plan and participate in health fairs, educational booths, presentations, workshops, and community outreach events. Develop educational materials, social media content, and marketing campaigns to promote public health programs and services. Administrative and Organizational Support Provide health education and community outreach programs by maintaining program documentation, tracking performance measures, preparing reports, coordinating educational events, managing educational materials and supplies, and assisting with grant reporting and program evaluation. Perform additional duties as assigned to support departmental operations and emerging public health priorities. Assist with coordination of outreach communications, educational materials, and social media content. Maintain accurate documentation and ensure confidentiality of all clients and program information in accordance with HIPAA and TRPHD policies. Organizational Participation and Compliance

• Comply with all TRPHD policies, procedures, and confidentiality requirements, including HIPAA regulations.
• Participate in assigned staff committees, required training, performance management, quality improvement activities, and accreditation efforts.
• Assist with emergency preparedness and response efforts during disease outbreaks, disasters, or public health emergencies as assigned.

• Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
• Ability to participate in and demonstrate health education and wellness activities as required by assigned programs. Work Environment

• Ability to work occasional evenings and weekends is based on programmatic or community needs.
