Job Description

Overview A part time Certified Diabetes Educator is needed in the women's health department of a multispecialty clinic in Monroe, NY 10950.
Schedule:
One day per week (6‑hour shift) -
Mondays or Thursdays Compensation:
$65 per hour

403(b) Retirement Plan, Short-term and Long-term disability, Life/Accidental Death and Dismemberment Insurance Responsibilities Patient Education — Provide comprehensive diabetes education and counseling to pregnant patients with gestational diabetes, including nutrition, medication management, insulin instruction, and lifestyle guidance. Care Coordination — Collaborate with physicians, nurses, and multidisciplinary team members to develop individualized care plans. Chronic Disease Management — Teach diabetes self‑management skills such as blood‑glucose monitoring, carbohydrate counting, and exercise planning. Support & Counseling — Provide ongoing emotional and behavioral support, helping patients maintain healthy habits and navigate challenges. Documentation — Record patient progress, interventions, and outcomes in the EMR and communicate updates to the care team. Follow‑Up Care — Conduct follow‑ups, adjust care plans, and ensure compliance with medications, lifestyle changes, and recommended check‑ups. Qualifications CDCES Certification or eligibility to sit for the exam. Gestational Diabetes Experience within an MFM department or Endocrinology practice. Strong knowledge of diabetes self‑management and patient education techniques. Excellent communication and interpersonal skills with a patient‑centered approach. Ability to work effectively in a collaborative, multidisciplinary environment. Pomera Care is a healthcare recruitment and consulting agency.
