Overview The Wellness Navigator is responsible for coordinating and facilitating evidence-based health and wellness programs that empower older adults to live healthier, more independent lives. This position serves as a guide and resource for participants by providing education, support, and encouragement while leading nationally recognized evidence-based classes and workshops. The Wellness Navigator builds meaningful relationships with participants, helps identify individual wellness goals, connects seniors with appropriate programs and community resources, and promotes healthy aging through education and engagement. Responsibilities include facilitating classes according to established program guidelines, maintaining required documentation and reporting, monitoring participant safety, assisting with program scheduling and outreach, and collaborating with staff and community partners to expand wellness opportunities. The ideal candidate is compassionate, organized, and passionate about improving the health and well-being of older adults. Strong communication and interpersonal skills, the ability to motivate and encourage participants, and a commitment to delivering high-quality, evidence-based programming are essential. Training and certification for required evidence-based programs will be provided as needed.

Job Summary/Duties:

Facilitate evidence-based health, wellness, and disease prevention programs in accordance with program fidelity requirements. Create a welcoming, inclusive, and supportive environment for all participants. Recruit, enroll, and orient participants into appropriate wellness programs. Assess participant needs and help connect individuals with available PCCOA programs, services, and community resources. Encourage healthy lifestyle choices and support participants in achieving personal wellness goals. Monitor participant safety during classes and respond appropriately to emergencies or health concerns. Maintain accurate attendance records, participant documentation, evaluations, and required program reports. Schedule classes and coordinate program logistics, including materials, equipment, and room setup. Help promote wellness programs through outreach events, presentations, newsletters, social media, and community partnerships when applicable. . Build and maintain positive relationships with participants, caregivers, healthcare providers, and community organizations. Maintain certification and continuing education requirements for evidence-based programs. Evaluate program effectiveness and recommend improvements to meet participant needs. Assist with grant reporting by data collection, and outcome measurements as required. Collaborate with other PCCOA departments to provide comprehensive services that support healthy aging. Assist with special events, health fairs, and wellness initiatives sponsored by PCCOA. Maintain confidentiality of participant information in accordance with agency policies. Follow all PCCOA policies, procedures, and safety guidelines. Perform other related duties as assigned.

Pay:

$12.84 - $19.26 per hour
