Job Summary:
Responsible for providing confidential assessments, individualized treatment plans, goal setting assistance, targeted coaching, health education, and self-care management tools to clients involved in a contracted employee wellness program. The RN Health Coach will make appropriate referrals to other programs that can assist the client in accomplishing their goals. In some instances, the RN Health Coach may work with the clients Primary Care Physician, Care Coordinators and Health Management resources within the employer's health plan.
EXPERIENCE DESCRIPTION
A minimum of 3 years of experience in inpatient and/or outpatient care, providing patient education is required. A minimum of 5 years of experience in inpatient and/or outpatient care, providing patient education is preferred. Experience in health and wellness coaching, patient advocacy, and/or case management is preferred.
EDUCATION DESCRIPTION
Associate's degree in Nursing is required. Bachelor's degree in Nursing is preferred.
TRAINING DESCRIPTION
Epic training within 90 days of hire is required.
SPECIAL SKILLS DESCRIPTION
Knowledge of local/federal regulations including HPIAA, and ADA. Knowledge of evidence based chronic condition management protocols. Effective oral and written communication. Working knowledge of motivational interviewing strategies. Ability to work independently and autonomously. Proficiency in Microsoft office suite and Publisher. Epic experience is preferred. Smoking Cessation counseling experience is preferred. Health Risk Appraisal software experience is preferred.
LICENSURE DESCRIPTION
Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from participating state in the NLC (Nurse Licensure Compact). Requires valid WI drivers license. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required. Certified Wellness Coach/Advocate from Wellcoaches or the National Society of Health Coaches (NCHS) is preferred.
