Duties Assist with daily resident and patient care activities, including personal hygiene, mobility assistance, and vital signs monitoring Administer medications and treatments accurately following physician orders and facility protocols Support specialized care needs such as Alzheimer's, dementia, memory care, and hospice services Operate electronic health record (EHR) systems like Epic and Cerner to document medical information securely and efficiently Perform basic medical procedures including tube feeding, catheter care, and wound management while maintaining strict adherence to HIPAA regulations Conduct assessments of residents' physiological status, including vital signs and overall health observations Ensure safe manual handling practices during heavy lifting or transfers to prevent injury to residents or staff Maintain detailed medical documentation and records in compliance with state healthcare regulations Communicate effectively with team members, residents, families, and healthcare providers to coordinate care plans Follow infection control procedures and uphold safety standards such as first aid, CPR certification, and emergency response protocols Skills Proven experience working with geriatric patients in assisted living, nursing homes, hospitals, or home care settings Strong knowledge of anatomy, physiology, medical terminology, and senior care best practices Familiarity with EMR/EHR systems like Epic and Cerner for accurate medical documentation Ability to perform patient care tasks including medication administration, tube feeding, catheter care, and vital signs monitoring Experience working with individuals with developmental disabilities or memory-related conditions such as Alzheimer's or dementia Knowledge of state Board of Nursing requirements and healthcare regulations governing patient safety and confidentiality (HIPAA) Skilled in heavy lifting, manual handling techniques, and safe resident transfers in compliance with safety standards Effective caregiver communication skills to collaborate seamlessly within multidisciplinary teams Certifications in first aid and CPR are required; additional certifications in hospice or home care are a plus Demonstrated ability to maintain patient confidentiality while managing sensitive medical records Join us to make a meaningful difference through dedicated caregiving!
