POSITION SUMMARY
Responsible for providing support for Specialty/OPD. Assists in identifying and meeting CRC referral and screening needs of community members over 50 years old. Works with the Specialty Clinic Staff, and OPD Referral and Logistics Coordinator in development and maintenance of patient care and referral database. Reports to the Ambulatory & Outpatient Services Manager or designee.
PRINCIPAL DUTIES AND RESONSIBILITIES
Schedules CRC screening tests that are recommended by the U.S. Preventive Services Task Force (USPSTF). Manages a patient tracking and reminder system for support screening adherence, the provision of appropriate and timely follow-up of all abnormal screening results and appropriate re-screening frequency. Develop and implement in-reach and out-reach strategies to promote screening. Uses appropriate data collection forms and/or site database to record outcome of all out/in-reach efforts, test(s) scheduled, screening outcomes, and follow-up and/or referral activities. Assists with Referral Services for Specialty and OPD Clinic as assigned , including, management of patient databases and pending follow-up. Procures equipment and supplies as indicated for patient care Participates in departmental and organizational performance improvement activities. Treats all patients and staff with dignity and respect. Schedule appointments at MHC and in Anchorage as directed by Medical Providers and Care Team. Assists with Specialty field clinics and scheduling of appointments and lodging as required. Provides good telephone etiquette, and displays good customer service. Also attends customer service when required. Identify and enroll patients into health insurance (i.e. Medicaid, other) if eligible, and obtains pre-authorizations when required. Maintain and disperse the annual Specialty Clinic Calendar Responsible for maintaining office supplies, and restocking/reordering for Specialty office Responsible for making provider travel/lodging arrangements Notifies MHC credentialing office of new providers in a timely manner
Job Qualifications:
MINIMUM REQUIREMENTS
High school diploma or equivalent; six months previous health service experience preferred. Knowledge of RPMS system preferred. Ability to utilize various computer programs and type well enough to carry out job duties efficiently. Must be able to maintain strict confidentiality at all times. Requires good interpersonal and communication skills. Requires strong organizational skills and ability to maintain file and computer database systems. Other Job Information (if applicable):
