We're looking for a compassionate, organized, and detail-oriented Patient Care Coordinator to join our growing team. In this role, you'll be the connection between patients, families, healthcare providers, and community partners, helping ensure everyone receives timely and coordinated care. What You'll Do As a Patient Care Coordinator, you'll serve as the primary point of contact for your assigned patient population and care teams.
Your day will include:
Coordinating patient care and follow-up services Managing incoming calls, emails, and patient inquiries Processing provider orders, including medications, labs, imaging, referrals, and home health services Communicating care plan updates with patients, families, community staff, and healthcare partners Supporting medication reconciliation and escalating concerns to providers when needed Assisting with post-discharge outreach and ensuring appropriate follow-up care is scheduled Identifying urgent patient concerns and helping coordinate timely interventions Maintaining accurate documentation and patient records Supporting telehealth appointments and care coordination activities What We're Looking For High school diploma or GED required Strong communication and customer service skills Comfortable using computers, email, and electronic medical records Ability to manage multiple priorities while maintaining attention to detail A proactive, team-oriented mindset with a passion for helping others Healthcare experience is a plus, but not required.
