Responsibilities Collaborate closely with healthcare professionals across hospitals, clinics, and post-acute care facilities to develop and implement individualized care plans. Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to inform case management strategies. Manage clinical case coordination for patients with complex needs such as critical care, trauma, infectious diseases, geriatrics, behavioral health, pediatrics, and post-acute recovery. Oversee discharge planning processes ensuring smooth transitions from inpatient to outpatient or home health settings while coordinating necessary services and resources. Utilize Electronic Health Records (EHR) systems efficiently to document patient information, medical records, and care activities in compliance with HIPAA regulations. Perform utilization management by evaluating diagnostic evaluations and managing hospital admissions or outpatient services to optimize resource use. Engage in patient service activities including client communication, education on health policies, and facilitating diagnostic or treatment procedures as needed. Requirements Valid RN license with current registration; must be able to demonstrate comprehensive nursing skills. Extensive hospital experience with critical care units such as

ICU, PICU

(Pediatric Intensive Care Unit), or Level I/II trauma centers preferred. Proven expertise in managing acute care cases across diverse settings including emergency rooms, medical-surgical units, outpatient clinics, and home health environments. Strong knowledge of health regulation policies including HIPAA compliance and medical terminology. Experience with EMR/EHR systems for accurate documentation and record management. Skilled in patient assessment techniques such as vital signs monitoring and physiological evaluations. Familiarity with discharge planning processes, post-acute care coordination, hospice services, and managed care principles. Ability to collaborate effectively with multidisciplinary teams while demonstrating excellent client communication skills. Knowledge of infection control protocols and experience managing infectious disease cases is advantageous. Exceptional organizational skills with the ability to handle multiple cases efficiently while maintaining attention to detail.
