Director of Case Management & House Supervisors

Job Description

The Opportunity Minden Medical Center is seeking a Case Management Director and House Supervisor to guide patient-centered care coordination and provide operational oversight across hospital units on days. This senior role supports safe, timely care transitions while partnering with physicians, nursing, and ancillary teams. Key responsibilities include leading case management processes, coordinating discharge planning and resource utilization, advocating for patients and families, facilitating collaboration and negotiation across services, and supervising hospital operations as needed to maintain quality outcomes and regulatory alignment.
Required qualifications/skills:
  • Unencumbered Louisiana RN license or Multi-State license
  • BSN preferred
  • 3 years hospital patient-care experience preferred; at least 1 year case management experience

A Typical Day Your day begins with a quick review of census, high-priority barriers, and overnight notes, then short touchpoints with nursing leaders and physicians to align on the plan for the shift. You'll spend time rounding to stay connected to patient and family needs, fielding real-time questions from staff, and keeping communication clear and respectful. Midday often includes reviewing documentation and key metrics, followed by coordinating with community resources and internal departments to keep plans moving. As the day wraps, you'll ensure smooth handoffs, confirm follow-ups are in place, and leave a clear status update for the next leader on duty.
