We follow a whole-child model that supports physical, mental, behavioral, developmental, and educational well-being. Through our Grow Well Center, we integrate behavioral health services directly within our pediatric setting to ensure coordinated, comprehensive care. Our team-based Collaborative Care Model (CoCM) emphasizes timely access, data driven, excellence, with provider engagement. Our practice culture values work-life balance. Our multidisciplinary team includes 3 MDs, 3 NPs, 3 LMHCs, 1 LABA/Educational Specialist, 1 Care Manager (CoCM), and a dedicated team of nurses, phlebotomist, medical administrators, and billing professionals. Our physicians are affiliated with Tufts Medicine and Lowell General Hospital. We maintain close working relationships with psychiatry colleagues at Tufts Medicine Integrated Network, as well as medical specialists both locally and in tertiary care settings. Behavioral Health Care Manager Position Overview The Behavioral Health Care Manager (BHCM) is a core member of our Collaborative Care Model (CoCM) team. This team will also include the patient's medical PCP, a psychiatric consultant, and licensed behavioral health clinicians (LHMC). The BHCM is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers. Responsible for carrying out effective care management for patients with behavioral health diagnoses, while also feeling comfortable with establishing new processes, communication systems, educational materials, and best practices alongside the behavioral health team and the providers. This person reports directly to the behavioral health lead. This position is in person, full-time (roughly 40 hours per week) with typical days and hours being Monday- Friday, 9am-5/6pm, Saturdays and evenings as needed with approval, paid lunch and at both practice locations (Chelmsford & Dracut) and Grow Well Center (Dracut). Essential Responsibilities Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient's medical provider and, when appropriate, other mental health providers. Screen and assess patients for common mental health and substance use disorders using validated screening tools. Facilitate patient engagement and follow-up care. Provide patient education about common mental health and substance use disorders and the available treatment options using approved psychoeducation materials. Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. Work with patients to assess and address identified Social Needs such as food insecurity, transportation, and housing needs. Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g., problem-solving treatment or behavioral activation) as clinically indicated. Provide coaching interventions to promote symptom awareness and symptom reduction. Coordinate with larger systems on patient's behalf, including when appropriate with schools, other medical providers, and/or family members. Participate in regularly scheduled (typically weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Caseload consultation reviews may be conducted by telephone, video, or in person. Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Document patient progress and treatment recommendations in EHR and other required systems to be shared with medical providers, psychiatric consultants, and other treating providers. In consultation with the other CoCM team members, LMHCs, & PCPs facilitate treatment plan changes for patients who are not improving as expected and/or who may need more intensive or specialized mental health care. Facilitate referrals for clinically-indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other interventions as appropriate. Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are preparing to be discharged from the caseload. Coordinate with existing clinic billing team members to bill for services. Perform all other duties and tasks as assigned to ensure efficiency and overall performance goals of the Practice. Adhere to the Practice employee handbook. Training & Licensure Requirements Bachelor's degree in social work, health and human services, psychology, or other physical health or mental health-related fields preferred. Equivalent experience will be considered in place of a degree on a case-by-case basis. The BHCM position does not require licensure. Qualifications Demonstrated ability to collaborate and communicate effectively in a team setting. Ability to maintain effective and professional relationships both with patients and other members of the care team. A desire to improve behavioral health access and outcomes through innovative programs. Ability to work with patients virtually as well as in person. Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation) under guidance of Licensed Professional. Understanding of HIPAA regulations and ability to work while maintaining confidentiality and privacy in accordance with governing HIPAA regulations.
