Position Overview The Community Outreach Coordinator is responsible for developing and maintaining relationships between Little Love Notes, LLC (LLN) and community organizations, referral sources, agencies, schools, courts, healthcare providers, treatment facilities, and other community partners. The Coordinator will actively represent LLN in the community by attending networking meetings, community events, outreach activities, and professional gatherings. The position is also responsible for managing LLN's social media presence and creating engaging content to increase community awareness, referrals, and participation in LLN programs and services. The Coordinator will identify new referral opportunities, establish community partnerships, increase awareness of LLN services, and support the organization's overall client census growth. Key Responsibilities Community Outreach & Networking Represent Little Love Notes, LLC professionally at community meetings, networking events, health fairs, conferences, workshops, and other outreach opportunities. Attend scheduled networking meetings and community events each month. Identify and participate in organizations and events that align with LLN's mental health, substance use disorder, PRP, therapy, medication management, and DUI education services. Build and maintain professional relationships with community stakeholders and referral sources. Provide information about LLN programs, services, eligibility requirements, referral procedures, and available appointments. Community Partnership Development Identify and establish new community partnerships with organizations that may refer clients to LLN. Develop relationships with schools, hospitals, healthcare providers, behavioral health agencies, residential programs, correctional facilities, courts, probation/parole offices, social service organizations, nonprofit organizations, and other community resources. Conduct outreach to potential referral partners through phone calls, emails, in-person visits, presentations, and networking activities. Coordinate presentations and informational sessions about LLN services for community organizations and referral sources. Maintain an updated database of current and prospective community partners. Follow up consistently with referral sources to strengthen relationships and identify additional collaboration opportunities. Census & Referral Development Develop and implement outreach strategies designed to increase referrals and support LLN's monthly client census. Work toward generating up to 25 new client referrals/admissions per month , based on program capacity, eligibility, payer requirements, and clinical appropriateness. Track referral activity, including referral source, referrals received, admissions, pending referrals, and outreach efforts. Follow up with prospective referral sources and organizations regarding referrals and available services. Collaborate with the Intake, Clinical, SUD, PRP, Therapy, and Administrative teams to support a smooth referral and admission process. Identify barriers to referrals and communicate trends and opportunities to LLN leadership. Social Media Management & Content Creation Manage and maintain LLN's social media presence across Instagram, Facebook, and YouTube . Develop and implement a consistent social media content calendar. Create engaging and professional content that promotes LLN's programs, services, community involvement, educational information, events, and organizational initiatives. Create and edit short-form videos, reels, posts, stories, graphics, and other digital content. Develop YouTube content, including educational videos, community highlights, interviews, program information, and other appropriate content. Capture photos and video footage during LLN events, networking activities, community outreach, and organizational activities. Create content that increases LLN's visibility and encourages potential clients, families, referral sources, and community partners to engage with LLN. Monitor social media comments, messages, engagement, and inquiries and respond professionally or route inquiries to the appropriate LLN staff member. Maintain consistent branding, messaging, and professional presentation across all LLN social media platforms. Promote upcoming LLN events, services, community partnerships, announcements, and educational opportunities. Monitor social media trends and identify opportunities to increase engagement and community reach. Track basic social media performance metrics, including reach, views, engagement, followers, inquiries, and content performance. Ensure all content complies with LLN policies, confidentiality requirements, and applicable privacy regulations. Obtain appropriate authorization before posting identifiable client photographs, testimonials, or other protected information. Marketing & Community Awareness Assist with the development and distribution of LLN outreach materials, brochures, flyers, service information, and referral materials. Promote LLN programs and services in appropriate community settings and online platforms. Assist with planning and participating in LLN-sponsored community events. Identify opportunities for LLN to provide educational presentations, workshops, and community resources. Collaborate with LLN leadership to develop campaigns designed to increase awareness and referrals. Collect community feedback regarding LLN services and communicate relevant information to leadership. Administrative Responsibilities Maintain accurate documentation of outreach activities, meetings, contacts, referrals, and community partnerships. Maintain an updated outreach and referral database. Submit monthly outreach, referral, census, and social media activity reports to management. Maintain a calendar of networking meetings, community events, presentations, and social media campaigns. Participate in staff meetings and organizational trainings as required. Maintain professional and timely communication with community partners and referral sources. Perform other duties related to community outreach, marketing, social media, and business development as assigned. Performance Expectations The Community Outreach Coordinator will be evaluated based on measurable outreach, marketing, partnership-development, and referral activities, including: Establishing new community partnerships. Maintaining relationships with existing referral sources. Attending networking meetings and community events. Generating qualified referrals. Supporting a target of up to 25 new client referrals/admissions per month , subject to LLN program capacity and client eligibility. Increasing awareness and visibility of LLN programs and services. Maintaining an active and consistent social media presence across Instagram, Facebook, and YouTube. Developing and publishing engaging, professional content on a consistent schedule. Increasing social media engagement, reach, and community awareness. Tracking outreach, referral, census, and social media metrics. Demonstrating consistent follow-up with referral sources and community partners. Contributing to overall organizational growth and community presence. Qualifications High school diploma or equivalent required; associate or bachelor's degree in marketing, communications, business, human services, social services, healthcare administration, or a related field preferred. Experience in community outreach, marketing, social media management, content creation, healthcare, behavioral health, social services, sales, business development, or referral coordination preferred. Experience managing Instagram, Facebook, and YouTube accounts preferred. Knowledge of social media content creation, video editing, photography, and graphic design preferred. Strong interpersonal, written, and verbal communication skills. Ability to establish and maintain professional relationships. Comfortable speaking with groups and representing an organization in the community. Strong organizational and follow-up skills. Creative, self-motivated, and goal-oriented. Ability to work independently and manage multiple outreach and content-creation activities. Reliable transportation and ability to travel throughout the community for meetings and events preferred.
