Case Management Lead

Job Description

Case Management Lead The Case Management Lead provides leadership and oversight for H.O.M.E., Inc.'s case management and navigation team, ensuring high-quality, trauma-informed support for individuals and families experiencing homelessness or housing instability. This role blends direct service with supervisory responsibilities, guiding staff in delivering compassionate, effective interventions that align with H.O.M.E., Inc.'s mission and values.

Responsibilities:

Supervise and support Case Management and Housing Navigation staff, providing regular coaching, training, and performance feedback. Oversee service planning processes to ensure consistency and quality across programs. Maintain a small caseload of clients to stay connected to direct service needs and realities. Collaborate with community partners, mental health providers, and housing agencies to coordinate care and expand resources. Ensure compliance with agency policies, confidentiality standards, and funding requirements. Lead team meetings and case reviews to promote collaboration and problem-solving. Track outcomes and prepare reports to inform program evaluation and improvement. Serve as a liaison between case management staff and senior leadership, advocating for client and staff needs. Participate in strategic planning and contribute to the development of new initiatives that strengthen H.O.M.E., Inc.'s service delivery.

Required Skills, Qualifications, and Physical Requirements:

Commitment to the mission and organizational values of H.O.M.E., Inc. Bachelor's Degree in Social Services or a Related Field Preferred At least three (3) years of case management, social work, or related human services experience with individuals facing mental health, substance use, and/or housing challenges. Current CPR & First Aid Certification (or will be provided) Demonstrated understanding of homelessness, poverty, and systemic barriers to housing stability. Familiarity with local and statewide mental health, housing, and supportive service systems Proven ability to use trauma-informed approaches to support clients through crisis. Strong leadership and mentoring skills with the ability to motivate and guide staff toward positive change. Excellent written and oral communication skills. Highly organized, able to manage multiple priorities, and maintain accurate documentation. Strong analytical and problem-solving skills. Ability to function well in a high-paced environment. Proficient with Microsoft Office Suite or related software. Valid driver's license, reliable transportation, and clean driving record. Must be able to remain in a stationary position (sitting or standing) up to 75% of the time. Must be able to ascend and descend stairs and get in and out of a vehicle frequently. Must be able to occasionally lift, carry, and move objects weighing up to 30 pounds. Background and license checks required prior to hire. Strong Candidates will demonstrate the following competencies: Supervisory experience managing, coaching, inspiring, and motivating staff to accomplish mission and goals by providing clear direction, accountability, and positive interpersonal influence. Successful partnering experience with social service program professionals, advocacy organizations, families, and community partners to achieve a common goal.
