Full-Time (32-40 hours per week) About Us Harding Place Assisted Living is a men's assisted living community operated by Horizon Health, Inc. Position Summary The Care Coordinator plays an important role in supporting residents' quality of life by coordinating appointments, transportation, activities, and community involvement. This position works closely with residents, healthcare providers, family members, and the On-Site Director to ensure residents receive the services and support they need while promoting independence, dignity, and engagement. Essential Duties and Responsibilities Schedule and coordinate medical, dental, therapeutic, and social service appointments for residents. Arrange and provide transportation for residents to appointments, community events, recreational outings, and other activities as needed. Assist residents with accessing community resources and services. Plan, organize, and facilitate resident activities that promote socialization, wellness, and quality of life. Encourage resident participation in community events and facility activities. Maintain accurate records of appointments, transportation schedules, and activities. Communicate effectively with residents, healthcare providers, family members, and staff regarding resident needs and schedules. Support daily operations and complete additional duties as assigned by the On-Site Director. Maintain confidentiality and comply with all facility policies and applicable regulations. Foster a positive, respectful, and resident-centered environment. QualificationsRequired High school diploma or equivalent. Valid driver's license with an acceptable driving record. Ability to safely transport residents. Strong organizational, communication, and time-management skills. Ability to work independently and as part of a team. Compassionate, patient, and professional approach to resident care. Preferred Previous experience in assisted living, healthcare, social services, human services, or customer service. Experience coordinating appointments, activities, transportation services, or community programs. Physical Requirements Ability to assist residents with entering and exiting vehicles safely. Ability to lift, push, pull, and carry up to 25 pounds. Ability to sit, stand, walk, bend, and reach throughout the workday. Ability to safely operate a motor vehicle.
