Community Health Representative (CHR)/Title VI Director

Job Description

Purpose The Community Health Representative (CHR) Program provides health care, health promotion and disease prevention services throughout the Trenton Indian Service Area to eligible program clients. The CHR Director will be responsible for the oversight of the CHR program and services provided. This position assumes responsibilities in providing quality customer and patient services in accordance with current clinical standards, policies and procedures while providing a commitment to quality healthcare and excellent customer service. This position will be full-time, Monday - Friday, 8:00 AM to 4:30 PM. Responsibilities
  • Direct department program including sharing responsibility in planning, organizing and implementing the CHR, Title VI and the Caregiver Assist programs for eligible Trenton Indian Service Area clients
  • Ensure compliance with program requirements and collaborate with appropriate company personnel as needed and required
  • Maintain client electronic records and data collection systems vital to tracking and documenting the program, services, and activities of the CHR program
  • Prepare a variety of monthly, annual and special reports on ongoing program activities
  • Identify and evaluate the needs of clients, develop, and implement intervention activities while assisting clients in accessing appropriate resources
  • Supports service area members and families with monitoring personal health and promoting preventative measures
  • Evaluate program operations in order to determine progress and problems regarding the delivery of client services
  • Recommend and implement operational changes and/or revision of policies as needed
  • Monitor, assess and approve expenditures within program funding sources
  • Facilitate upkeep and repairs of all staff assigned GSA vehicles
  • Travel required within service area: conducting home visits, providing health education and maintaining health surveillance; transportation is provided for service area clients to and from medical appointments; prescription delivery as needed and scheduled; coordinate other transportation needs as they arise in regards to caring for eligible service area members; deliver senior meals to clients who are homebound, etc.
  • Additional travel may be required for other services, trainings, seminars, etc.
  • Research, arrange and manage appropriate travel accommodations for clients obtaining medical care outside of defined service area
  • Organize, develop and facilitate program sponsored events
  • Direct department program and oversee, train, educate CHR personnel
  • Communicate relevant and necessary information to appropriate staff while adhering to confidentiality standards

  • Regularly required to stand, bend, talk, hear and lift up to 20+ pounds
  • Ability to withstand extended periods of travel in work provided vehicles, with occasional overnight travel
  • Fine motor skills
  • Visual acuity
  • Involves direct and frequent contact with clients Qualifications
  • High school diploma or GED equivalent
  • Minimum of two (2) years experience in similar CHR or relatable IHS or healthcare program
  • Minimum of two (2) years supervisory experience in similar CHR or relatable IHS or healthcare program preferred
  • Current CPR/First Aid Certification preferred
  • Valid driver's license with acceptable driving record
  • Ability to collect, summarize and report data coherently and logically
  • Advanced computer skills to sufficiently input and record client and program data; prepare reports as required and requested
  • Demonstrate awareness, sensitivity, and appreciation of Native American culture, traditions, customs and needs
  • Grant writing experience or skills with previous administration of federal, state and/or local grants
  • Excellent interpersonal and communication skills and empathetic in client interactions
  • High standards of ethics and confidentiality when handling sensitive information Ability to successfully pass all pre-employment requirements including a drug/alcohol test and background investigation -Additional responsibilities may be assigned as appropriate, this summary is not considered to encompass all responsibilities
Pay:
$20.00 - $22.00 per hour
