40 hours per week (Monday through Friday), including participation in the Administrative On-Call rotation (5PM - 8AM) About the
Job:
The Clinical Manager provides leadership and operational oversight for a multidisciplinary Home Health and/or Hospice team. Reporting to the Chief Operating Officer, this role ensures high-quality patient care, regulatory compliance, staff development, care coordination, and achievement of organizational goals. The Clinical Manager serves as a key resource for clinicians while driving quality outcomes, productivity, and exceptional patient experiences.
What You'll Do:
Lead and supervise a team of nurses, therapists, and clinical staff. Facilitate interdisciplinary team meetings and case conferences to support coordinated patient care. Provide guidance and support to case managers regarding patient care, documentation, and care planning. Review patient progress, recertification plans, visit frequency, and discharge readiness. Monitor clinical quality, outcomes, utilization, and productivity metrics. Partner with Quality, Human Resources, and Staff Development teams to support performance improvement, employee engagement, onboarding, and retention. Ensure compliance with Medicare Conditions of Participation, infection prevention standards, and agency policies. Analyze operational data and identify opportunities to improve patient outcomes, efficiency, and staff performance. Assist with recruitment, interviewing, and selection of new clinical team members. Manage staffing assignments, overtime utilization, and departmental resources.
Who You Are:
Registered Nurse licensed in New Hampshire with an Associate's Degree in Nursing, or a Master's Degree in Therapy or Social Work. Minimum of three (3) years of recent acute care or post-acute care experience. Experienced clinical leader with prior supervisory or management experience. Knowledgeable in Home Health and/or Hospice regulations, Medicare Conditions of Participation, and clinical best practices. Skilled in team leadership, coaching, performance management, and interdisciplinary collaboration. Strong analytical, organizational, communication, and problem-solving skills. CPR certified with a valid driver's license and proof of insurance. Home Health, Hospice, OASIS, Allscripts EMR, Oncology, or Community Health experience is a plus.
