About Hicare
Hicare is a Digital Healthcare Solution and Service company specializing in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and value-based healthcare solutions. We partner with physician practices, healthcare organizations, and health plans to improve patient outcomes and help providers deliver high-quality care beyond traditional office visits.
Position Overview
Hicare is establishing its New Jersey operations to serve providers and Medicare patients within a contracted medical group.
We are seeking a dependable and motivated Digital Healthcare Manager who can help build this operation from its early stages and grow with the company. Initially, the manager will work directly with patients and clinics while helping establish practical workflows and a strong service foundation. As the number of participating providers and patients grows, the role will gradually expand to include building, training, and managing a local service team. This position is well suited for someone who takes ownership, follows through on commitments, and is willing to build an operation steadily rather than expecting everything to be fully established from day one. Hicare will provide support, systems, and guidance, while entrusting the manager with increasing responsibility as the New Jersey operation develops.
Key Responsibilities
1.
Program Operations and Coordination Develop a practical understanding of Hicare's care programs and oversee their day-to-day delivery in New Jersey. Coordinate patient enrollment, care services, documentation, and communication among Hicare staff and participating clinics. Monitor overall workflow and promptly address operational issues. Provide direct patient support when needed, particularly during the initial stage of the New Jersey operation. 2. Staff Management and Service Quality Supervise and support care coordination staff responsible for delivering patient services. Assign and monitor workloads, patient follow-ups, documentation, and service completion. Ensure that staff understand program requirements and perform their responsibilities consistently. Provide training and feedback to maintain service quality and accountability. 3. Provider and Clinic Relationship Management Serve as Hicare's primary operational contact for providers in New Jersey. Communicate with clinic personnel to review service status and operational concerns. Build trusted, long-term relationships by ensuring that providers receive responsive and reliable service from Hicare. 4. Performance, Compliance, and Growth Monitor patient participation, service completion, documentation quality, staff performance, and clinic satisfaction. Ensure that services and documentation comply with Medicare requirements, provider instructions, HIPAA, and company policies. Provide regular operational updates to Hicare management and recommend workflow or staffing improvements. Support the gradual expansion of services by helping recruit, train, and manage additional staff as the New Jersey operation grows.
Qualifications
Required Minimum of two years of experience working in a clinic or other healthcare setting.
Strong care coordination and patient communication skills. Excellent organizational, leadership, problem-solving, and documentation skills. Ability to establish and maintain professional relationships with physicians, clinic managers, and healthcare staff. Bilingual proficiency in English and Korean. Preferred Experience with Chronic Care Management (CCM). Experience working with Medicare patients. Experience in primary care, internal medicine, family medicine, or home health. Experience using EMR/EHR systems and other healthcare technology platforms. Experience helping establish or expand a healthcare service, team, or local operation.
