Care Manager

Job Description

Are you passionate about mental health and helping children and families succeed? Tri County Care Management Organization (TCCMO) is a non-profit organization that provides care management (case management) services for children with complex challenges in Hunterdon, Somerset and Warren Counties. We are currently seeking a Care Manager based in our Stewartsville, NJ office to provide support to youth who are experiencing behavioral, emotional, social, developmental, and/or mental health challenges. This is a full-time position working primarily with youth in Hunterdon County. Our Care Managers play a pivotal role in guiding youth and their families in the right direction to meet their goals and thrive by providing intensive wraparound services that help youth succeed at home, in school, and in the community. Responsibilities of a
Care Manager:
Prepare and carry out short-term crisis plans and Individualized Service Plans (ISP) as needed Source resources for youth to access formal and informal services Act as liaison between families and providers Travel to various meetings at homes, schools, courts, and attend other services with the youth and families as needed Advocate for youth and families in those meetings as needed Follow established timelines for meetings, calls, documentation, and plan submission Maintain accurate electronic health records and youth files Participate in required training opportunities with the option of additional training to enhance skills and further education
Position Requirements:
Bachelor's degree in social work, psychology or related discipline At least one (1) year of experience in a child service system environment Strong interpersonal and organizational skills Experience with DD/ID population preferred Bilingual skills desirable Ability to travel throughout
