Summary As a Medical Coordinator at ACDS, Inc., you will partner closely with our nurses to ensure that our residents with special needs receive top-quality care that is complete and correct. You will also play a vital role in ensuring the smooth operation of their healthcare services. This position is essential in supporting our mission to provide exceptional care and to help individuals reach their fullest potential. $1,000 SIGN-ON
BONUS FOR ALL NEW HIRES
(For employees working a minimum of 16 hours per week): $500 awarded upon successful completion of the 90-day introductory period in good standing. An additional $500 awarded after 6 months of employment. Responsibilities Coordinate medical appointments and ensure timely follow-up on health-related issues for residents. Accompany residents on medical appointments. Review and maintain accurate documentation of medical records and resident information. Provide administrative support to the RNs within the Agency. Track and monitor all prescriptions and medications for residents, call in refills as needed, communicate changes in medications to the RN. Administer medication to residents. Perform other tasks as assigned. Qualifications High School Diploma or equivalent Valid New York State Driver's License Valid AMAP certificate Prior experience working with individuals with developmental disabilities, preferred. Minimum of 6 months experience in a direct support role Ability to communicate effectively with families and healthcare professionals.
