Care Manager - Adult Home (75 Beds)

Pay:

$25-$28 per hour (based on experience) We are seeking a compassionate, organized, and dedicated Care Manager to join our team at a 75-bed adult home. The ideal candidate will have strong communication skills, leadership ability, and a passion for helping residents maintain the highest possible quality of life.

Responsibilities include:

Coordinate and oversee resident care plans and services Communicate with residents, families, healthcare providers, and staff Monitor residents' needs and ensure proper follow-up Assist with admissions, assessments, and ongoing resident support Maintain accurate documentation and records Coordinate medical appointments, services, and outside providers Help ensure compliance with facility policies and regulatory requirements Work closely with administration and department leaders to support daily operations

Qualifications:
