TriSource is seeking an experienced Registered Nurse Supervisor to oversee and support nursing services in a home care setting for a client in Austin, TX. This role is responsible for ensuring high-quality, client-centered care while providing clinical guidance and support to RNs, LPNs, PCAs, and other field staff. The RN Supervisor will work closely with clients, families, physicians, case managers, therapists, and internal teams to ensure continuity of care, compliance with care plans, and the highest standards of clinical practice. Key Responsibilities Oversee daily nursing operations and ensure compliance with home health regulations and standards. Provide clinical supervision, guidance, and education to RNs, LPNs, PCAs, and field caregivers. Conduct initial assessments, supervisory visits, and ongoing client evaluations. Develop, review, and update individualized care plans based on client needs. Monitor medications, treatments, nursing documentation, and overall quality of care. Monitor changes in client condition and communicate concerns, physician orders, and clinical updates to the appropriate healthcare providers. Collaborate with physicians, case managers, families, clients, and interdisciplinary healthcare teams to ensure continuity of care. Provide direct nursing care and clinical support to clients and field staff as needed. Ensure staff follow established safety, medication, infection-control, and clinical procedures. Support staff performance evaluations, coaching, and corrective action in collaboration with HR. Monitor incidents and assist with appropriate investigation and follow-up. Participate in regulatory surveys, inspections, meetings, and required documentation. Travel regularly to client locations and maintain consistent communication with field staff and clients. Qualifications Current Registered Nurse (RN) license in the applicable state. Minimum of 2 years of clinical or home health nursing experience. Experience working with individuals who have chronic, complex, rehabilitative, or catastrophic medical needs is preferred. Bachelor's degree in Nursing (BSN) required Rehabilitation nursing or related specialty certification preferred. Strong clinical judgment, assessment, problem-solving, and decision-making skills. Ability to effectively communicate and collaborate with clients, families, physicians, case managers, and interdisciplinary teams. Valid driver's license, automobile insurance, and an acceptable driving record required. Ability to travel regularly to client locations.
