Full-Time Job Summary The State of Tennessee Department of Health is seeking a Medical Social Worker - Patient Care Coordinator to support primary care clinics and improve patient access to healthcare and community resources. The Medical Social Worker will work as part of a Clinical Care Team, collaborating with healthcare providers, nurses, community organizations, and patients to address social determinants of health (SDOH) such as food insecurity, housing needs, transportation barriers, financial challenges, and access to medical services. This position provides patient advocacy, care coordination, resource navigation, and case management support to help patients achieve better health outcomes. Key Responsibilities Conduct patient assessments to identify social, medical, and psychosocial needs. Provide social support navigation for patients experiencing barriers related to: Food insecurity Housing instability Transportation challenges Financial concerns Healthcare access Mental health and social service needs Connect patients with appropriate community resources and support programs. Develop and maintain resource directories, including eligibility requirements, referral processes, and contact information. Collaborate with primary care providers, nurses, and clinical staff to support patient care plans. Provide case management services, including: Hospital discharge planning and follow-up Care plan monitoring Patient outreach and follow-up Assistance with obtaining medical records Support for patients overdue for medical care Assist patients with specialty referral coordination, including scheduling appointments and tracking referrals. Help patients access affordable medication programs, prescription assistance programs, and healthcare resources. Provide patient education and connect individuals with appropriate health education materials. Document patient interactions, referrals, assessments, and care coordination activities in medical records. Participate in provider meetings, community health meetings, and regional healthcare initiatives. Travel within the region may be required as needed.
Required QualificationsEducation & Experience:
Social Worker Candidates:
Bachelor's, Master's, or Doctorate degree in Social Work from an accredited college or university. Minimum of 1 year of professional social work or related experience. OR Possession of a Master of Social Work Certificate (CMSW) or Licensed Clinical Social Worker (LCSW) credential from the State of Tennessee.
Social Counselor Candidates:
Bachelor's degree from an accredited college or university. Minimum of 1 year of professional experience in one or more of the following areas: Healthcare services Social work Social or psychological counseling Mental health or behavioral rehabilitation counseling Early intervention/service coordination Medical, educational, or social services settings Required Skills & Experience Minimum 1 year of experience with social support navigation related to Social Determinants of Health (SDOH) , including: Food insecurity assistance Housing resource coordination Community service referrals Patient advocacy Experience working with healthcare teams, patients, or community organizations. Strong communication, documentation, and case management skills. Ability to assess patient needs and coordinate appropriate services. Knowledge of local healthcare and social service resources.
