Job Overview We are seeking a dedicated and detail-oriented Clinical Navigator to join our team in a full-time capacity during day shifts at our rehabilitation facility. The Clinical Navigator will serve as a vital link between patients, families, and healthcare providers, ensuring seamless care coordination and optimal patient outcomes. This role requires a strong clinical background, excellent communication skills, and a passion for patient-centered care. Duties Coordinate multidisciplinary care plans by collaborating with medical staff, therapists, and support teams to meet individual patient needs. Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations to inform discharge planning and ongoing treatment strategies. Manage electronic health records (EHR) documentation accurately while ensuring compliance with HIPAA regulations and hospital policies. Facilitate effective communication with patients and families to explain treatment options, discharge instructions, and post-acute care plans. Oversee utilization management processes to ensure appropriate resource use while maintaining high standards of patient service and safety. Assist in managing patients as a nurse case manager by coordinating diagnostic evaluations, medical records review, and post-discharge follow-up. Support infection control protocols and maintain awareness of health regulation policies relevant to acute care environments. Experience Candidates should possess substantial clinical experience within hospital or rehabilitation settings, including critical care, ICU, or emergency medicine environments. Prior experience in Level I or Level II trauma centers is highly desirable. A background in hospital medicine, infectious disease care, pediatrics (PICU), geriatrics, behavioral health, or medical-surgical units will strengthen candidacy. Knowledge of hospital experience standards such as HIPAA compliance, medical terminology proficiency, discharge planning procedures, and electronic health records management is essential. Applicants should demonstrate expertise in patient assessment techniques, managing patients as a nurse case manager or in outpatient clinics, along with acute care experience including intake procedures and utilization management. Experience in post-acute or outpatient care settings is preferred for a comprehensive understanding of the continuum of patient recovery.
