Job Description

Liberty Health seeks an experienced MDS Coordinator to manage the Resident Assessment Instrument (RAI) and Minimum Data Set process within our Healthcare & Medical Services department. In this role, you'll ensure accurate, timely MDS completion, optimize reimbursement, and support regulatory compliance while collaborating with interdisciplinary teams to develop and monitor individualized care plans. You'll analyze quality measures, educate staff on MDS and documentation standards, and champion patient-centered care.
