Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care for one client across diverse needs. In this vital role, you will support individuals with their daily activities, promote independence, and enhance their quality of life through personalized care. Your energy, attention to detail, and empathetic approach will make a meaningful difference in the lives of those you serve. This position offers an opportunity to work in a dynamic environment where your caregiving skills directly impact patient well-being and comfort. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure their comfort and dignity. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormalities promptly. Support medication administration following prescribed instructions while maintaining accurate medical documentation in compliance with home healthcare regulations. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and safety measures. Facilitate patient transfers using proper methods such as Hoyer lifts or transfer belts, ensuring safety for both client and caregiver. Maintain patient confidentiality by adhering to HIPAA guidelines and safeguarding medical records and personal information. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with client needs. Skills Proven experience working with individuals with disabilities, geriatrics, or in assisted living environments. Strong knowledge of medical terminology, home health care assistance protocols, and home health agency standards. Competence in operating EMR (Electronic Medical Records) systems and documenting medical records accurately. Excellent patient communication skills with the ability to provide compassionate personal care assistance. Ability to perform heavy lifting safely and operate equipment like Hoyer lifts or other patient transfer devices. Knowledge of hospice care principles and end-of-life support practices. Certifications in First Aid, Basic Life Support (BLS), CPR, and medication administration are highly desirable.
