In this vital role, you will provide compassionate, personalized care to patients in their homes, supporting their health, safety, and comfort. Your energetic approach and attention to detail will make a meaningful difference in the lives of individuals with diverse needs, including seniors, those with disabilities, and patients requiring end-of-life care. This position offers a rewarding opportunity to empower clients through quality home health services while fostering independence and dignity. Duties Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates. Administer medications and perform catheter care following medical instructions and home health care regulations. Observe patient conditions closely, noting changes in health status or behavior related to dementia, Alzheimer's disease, or other cognitive impairments. Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while ensuring HIPAA compliance and patient confidentiality. Support clients with meal preparation, light housekeeping, laundry, and cleaning to promote a safe and comfortable environment. Communicate effectively with patients, families, and healthcare teams to coordinate care plans and update on patient progress or concerns. Skills Proven experience working with geriatric patients and individuals with developmental disabilities or dementia. Strong knowledge of home health care assistance techniques including assisting with ADLs and patient transfer methods. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Ability to perform medical procedures such as vital signs measurement, medication administration, catheter care, and basic life support (BLS) or CPR certification. Excellent patient observation skills to detect changes in condition or behavior promptly. Understanding of home health regulations, HIPAA compliance, and maintaining patient confidentiality at all times. Physical stamina for heavy lifting and assisting with mobility tasks; experience working with Hoyer lifts preferred.
