Overview We are seeking a compassionate, dependable, and professional Certified Home Health Aide (HHA) to provide quality personal care and supportive services to patients in their homes. The HHA will assist patients with activities of daily living while promoting safety, dignity, independence, and comfort. The ideal candidate is reliable, caring, patient, and able to work independently while following the patient's care plan and agency policies. Responsibilities Provide personal care and assistance with activities of daily living (ADLs) Assist patients with bathing, grooming, dressing, toileting, and personal hygiene Assist with ambulation, transfers, positioning, and use of assistive devices as appropriate Provide assistance with meal preparation and feeding when needed Assist with light housekeeping related to the patient's care and living area Assist with medication reminders according to the patient's care plan and agency policy Monitor and report changes in the patient's condition to the supervising nurse or appropriate clinician Maintain a safe, clean, and comfortable environment for the patient Follow the patient's individualized plan of care and agency procedures Maintain patient privacy and confidentiality Document services provided accurately and timely Communicate professionally with patients, families, caregivers, and the home health team Support patients in maintaining independence and quality of life Qualifications Current Home Health Aide (HHA) certification as required by state regulations Previous home care or healthcare experience preferred Reliable transportation to patient homes Ability to communicate effectively with patients, families, and healthcare professionals Ability to safely assist with transfers, ambulation, and personal care Compassionate, dependable, and respectful Ability to work independently and follow a plan of care Must be able to pass required background checks and meet agency employment requirements Join us in providing compassionate home health services that empower individuals to live safely and comfortably at home!
