Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support

MUST BE ABLE TO LIFT 50+ POUNDS

(assist with patients) Monitor patient health by observing vital signs and recognizing changes in condition Administer medications and perform catheter care following medical instructions and home healthcare regulations Maintain accurate medical documentation using Electronic Health Record (EHR) systems and ensure compliance with HIPAA privacy standards Support clients with memory care needs, dementia, Alzheimer's disease, and hospice or end-of-life care with patience and empathy Utilize patient transfer methods including Hoyer lifts and safe lifting techniques for heavy lifting tasks Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and update progress Skills Extensive experience working with geriatric patients, individuals with developmental disabilities, or those requiring specialized dementia or Alzheimer's care Proficiency in using EMR (Electronic Medical Records) systems for documentation and medical records management Strong knowledge of home health care assistance, including assisting with ADLs, medication administration, and vital signs monitoring Familiarity with home health regulations, patient confidentiality (HIPAA), and safety protocols such as first aid and CPR/Basic Life Support (BLS) certification Ability to perform patient transfers using methods like Hoyer lifts or other transfer techniques safely and efficiently Experience working within home health agencies or providing live-in employment services preferred Excellent communication skills for client interaction, caregiving documentation, and team collaboration Join us in delivering exceptional home healthcare that empowers clients to live comfortably and safely in their own homes.
